结直肠癌肝转移的危险因素分析

ANALYSIS OF RISK FACTORS FOR LIVER METASTASES OF COLORECTAL CANCER

目的:分析影响结直肠癌肝转移的危险因素。方法:回顾性分析154例结直肠癌病人临床资料,采用logistics回归分析法分析影响结直肠癌肝转移的独立危险因素。结果:154例病人中,71例发生肝转移(46.10%),83例未发生肝转移(53.90%);原发瘤分化程度、CEA、原发瘤区域淋巴结转移、肝转移瘤切缘、是否输血是影响结直肠癌病人肝转移单因素(P<0.05);其中原发瘤中低分化、CEA≥5ng/mL、原发瘤区域淋巴结转移和术中输血是影响结直肠癌病人肝转移独立危险因素(OR=1.265、1.357、1.411、1.520,P<0.05)。结论:原发瘤中低分化、CEA≥5ng/mL、原发瘤区域淋巴结转移、术中输血是影响结直肠癌病人肝转移的独立危险因素,临床治疗中可综合上述独立危险因素,合理评估病人病症。

Objective: To analyze the risk factors affecting liver metastases of colorectal cancer. Methods: The clinical data of 154 patients with colorectal cancer were retrospectively analyzed.Logistic regression analysis was used to analyze the independent risk factors affecting liver metastases of colorectal cancer. Results: Among the 154 patients,71 cases( 46.10 %)were with liver metastases and 83 cases( 53.90 %)without liver metastases.Primary tumor differentiation degree,CEA, lymph node metastasis in primary tumor area,incisal margin of liver metastases and blood transfusion or not were single factors affecting liver metastases in patients with colorectal cancer( P < 0.05 ).Among them,low differentiation of primary tumor,CEA≥5 ng/mL,lymph node metastasis in primary tumor area and intraoperative blood transfusion were independent risk factors for liver metastases in patients with colorectal cancer( OR = 1.265 、 1.357 、 1.411 、 1.520 , P < 0.05 ). Conclusion: Low differentiation of primary tumor,CEA≥5 ng/mL,lymph node metastasis in primary tumor area and intraoperative blood transfusion are independent risk factors for liver metastases in colorectal cancer patients.The above independent risk factors can be integrated in clinical treatment to reasonably evaluate disease of patients.
