无张力疝修补术治疗腹股沟疝的临床效果研究

Study on the Clinical Effect of Tension-Free Herniorrhaphy for Inguinal Hernia

目的分析无张力疝修补术在治疗腹股沟疝方面的临床治疗效果。方法研究对象方便选择该院2016年1月—2018年1月所收治的86例成人腹股沟疝患者,并随机将其分为观察组与对照组,每组43例。对照组选择传统疝修补术,观察组选择无张力疝修补术,在手术结束后比较两组患者的手术时间、术中出血量、肛门排气时间、下床活动时间、术后住院时间几项指标,结合并发症情况进行对比。结果观察组患者在手术时间、术中出血量、肛门排气时间、下床活动时间与术后住院时间几项围术期指标对比方面均要优于对照组;并发症方面,观察组出现了1例腹腔出血与2例切口感染。对照组出现2例阴囊血肿、2例腹腔出血、2例切口感染与1例复发,发生率上观察组7.0%也要明显低于对照组的16.3%。两组患者数据差异有统计学意义(χ^2=6.480,P<0.05)。结论无张力疝修补术的术中输血量较少,相比于传统疝修补术可以具有控制疼痛的作用,患者预后恢复速度较快,我们可以认为成人腹股沟疝的治疗方案可以选择无张力疝修补术。

Objective To analyze the clinical effect of tension-free hernia repair in the treatment of inguinal hernia.Methods A total of 86 adult inguinal hernia patients admitted to our hospital from January 2016 to January 2018 were convenient selected as subjects and randomly divided into observation group and control group,43 cases in each group.Traditional herniorrhaphy was used in the control group,and tension-free herniorrhaphy was used in the observation group.After the operation,the operation time,intraoperative blood loss,anal exhaust time,bed movement time,postoperative hospitalization time,and the complications of the two groups were compared.Results The observation group was superior to the control group in the comparison of perioperative indexes such as operation time,intraoperative blood loss,anal exhaust time,time of getting out of bed and postoperative hospitalization time.In terms of complications,there was 1 case of abdominal bleeding and 2 cases of incision infection in the observation group.There were 2 cases of scrotal hematoma,2 cases of abdominal bleeding,2 cases of incision infection and 1 case of recurrence in the control group.And the incidence of in the observation group(7.0%)was significantly lower than that in the control group(16.3%).The data of patients in the two groups were significantly different,with statistical significance(χ^2=6.480,P<0.05).Conclusion The amount of intraoperative blood transfusion for tension-free herniorrhaphy is less.Compared with traditional herniorrhaphy,tension-free herniorrhaphy can control pain,and the prognosis and recovery rate of patients are faster.Therefore,it can be considered that tension-free herniorrhaphy can be used for the treatment of adult inguinal hernia.
