腰丛-坐骨神经联合阻滞对股骨上段骨折手术麻醉的临床效果研究

Clinical effect of lumbar plexus-sciatic nerve block on surgical anesthesia of upper femoral fracture

目的分析腰丛-坐骨神经联合阻滞对股骨上段骨折手术麻醉的临床效果,探究腰丛-坐骨神经联合阻滞在临床上的应用意义。方法选择本院2015年3月至2017年3月收治的84例股骨上段骨折患者作为研究对象,根据患者麻醉方式的不同分为两组,各42例,对照组给予硬膜外麻醉,实验组给予腰丛-坐骨神经联合阻滞治疗,比较麻醉效果。结果麻醉后,对照组的血流动力学指标明显下降,与麻醉前比较差异有统计学意义(P<0.05)。另外,麻醉后,对照组患者的血流动力学指标显著低于实验组,差异有统计学意义(P<0.05)。对照组并发症发生3(7.14%)例,实验组并发症发生0例(0.00%),两组经过麻醉后实验组患者感觉神经阻滞时间长于对照组,运动神经阻滞短于对照组(P<0.05)。结论对于股骨上段骨折的患者在进行治疗的过程中,采用腰丛-坐骨神经联合阻滞麻醉具有较好的麻醉效果,值得进行推广和应用。

Objective To analyze the clinical effect of combined lumbar plexus-sciatic nerve block for anesthesia of femoral fractures and explore the clinical significance of lumbar plexus-sciatic nerve block.Methods 84 patients with femoral fractures treated in our hospital from March 2015 to March 2017 were selected as study subjects.The patients were divided into two groups according to the different ways of anesthesia,with 42 cases in each group.Epidural anesthesia was performed in the control group.The experimental group was treated with combined lumbar plexus-sciatic nerve block and the anesthetic effect was compared.Results After anesthesia,the hemodynamic parameters of the control group decreased significantly,and the difference was significant(P<0.05).In addition,after anesthesia,the hemodynamic parameters of the control group were significantly lower than the experimental group,and the difference was significant(P<0.05).There were 3(7.14%)cases of complication in the control group and 0 cases(0.00%)in the experimental group.After the anesthesia,the sensory nerve block in the experimental group was longer than that in the control group,and the motor block was shorter than the control group(P<0.05).Conclusion For the treatment of upper femoral fracture patients,the combination of lumbar plexus-sciatic nerve block anesthesia has good anesthetic effect and is worthy of promotion and application.
