术中B超检测脑脊液动力学在Chiari畸形Ⅰ型改良术式中的应用 被引量:1

摘要 目的探究术中B超检测脑脊液动力学在Chiari畸形Ⅰ型改良术式中的应用效果。方法选取本院60例Chiari畸形Ⅰ型患者,根据术中脑脊液动力学分组,A组(n=10)脑脊液峰值流速≥3.0cm/s,行后颅窝骨性减压术;B组(n=50)脑脊液峰值流速<3.0cm/s,行改良后颅窝骨性减压硬膜成形术。对比两组神经功能改善、术后脊髓空洞变化情况及B组硬膜成形术前后脊髓背侧脑脊液流速峰值。结果A组神经功能改善率100.00%(10/10)与B组80.00%(40/50)对比,差异无统计学意义;B组术后脊髓背侧脑脊液流速峰值高于术前,差异有统计学意义(P<0.05);MRI术后检查示,A组脊髓空洞缩小率100.00%(10/10)与B组脊髓空洞缩小率96.00%(48/50)对比,差异无统计学意义。结论两组均行后颅窝骨性减压术,术中B超检测脑脊液动力学,对骨性减压后脑脊液峰值流速<3.0cm/s的Chiari畸形Ⅰ型患者行改良硬膜成形术治疗,可有效改善患者脑脊液动力学,缩小脊髓空洞,促进患者神经功能恢复。
