高频振荡与同步间歇指令通气治疗严重新生儿呼吸窘迫综合征的比较分析 被引量:3

摘要 目的对比高频振荡与同步间歇指令通气治疗NRDS的临床疗效。方法研究对象为2014年1月~2018年12月期间59例诊断为重度NRDS的早产儿,入院后随机分组,A组采用高频振荡,B组采取同步间歇指令通气治疗,比较两组临床效果。结果A组、B组呼吸机治疗前PaO2、PaCO2、FiO2、MAP指标差异无统计学意义(P>0.05),治疗后A组PaO2值高于B组,A组1h、6h、12hPaCO2、FiO2低于B组,A组通气后1h、6h、12hMAP高于B组,差异有统计学意义(P<0.05),两组24h时PaO2、PaCO2、FiO2、MAP差异无统计学意义(P>0.05)。两组机械通气时间、存活例数差异无统计学意义(P>0.05)。结论通过对严重NRDS患儿采用高频振荡、同步间歇指令通气均可获得良好疗效,高频振荡可较快改善氧合,改善肺功能,研究结果有一定参考价值。 Objective Compared the clinical curative effect between high-frequency oscillation ventilation and synchronized intermittent aeration instruction ventilation treatment of severe NRDS. Methods From January 2014 to December 2018, 59 cases were diagnosed as premature NRDS, and they were randomized divided into two groups after admission. Group A was treated with high frequency oscillation, and group B was treated with synchronized intermittent aeration instruction. Compared the clinical effect between the group A and group B. Results There were no statistically significant differences in PaO2, PaCO2, FiO2 and MAP indexes between group A and group B before ventilator treatment (P > 0.05). PaO2 value in group A was higher than group B after treatment, and PaCO2, FiO2 in group A was lower than that in group B in 1h, 6h, 12h after ventilator treatment. The difference of MAP between the two groups was statistically significant (P < 0.05). The difference in PaO2, PaCO2, FiO2, MAP between the two groups at 24h was not statistically significant (P > 0.05). Conclusion High frequency oscillation and synchronous intermittent command ventilation can achieve good efficacy for children with severe NRDS. High frequency oscillation can rapidly improve oxygenation and lung function, and the research results have certain reference value.
