应用血栓弹力图分析血糖控制与阿司匹林抵抗的关系

摘要 目的应用血栓弹力图分析探讨2型糖尿病老年患者阿司匹林抵抗的发生情况及相关机制。方法将2018年1月-2018年10月于南阳市骨科医院治疗的160例2型糖尿病患者作为研究对象,均服用拜阿司匹林100mg/d,>7d,根据血栓弹力图(TEG)检测全部患者血小板聚集程度计算阿司匹林敏感(AS)、阿司匹林半抵抗(ASR)组及抵抗(AR)组发生率,对比分析各组血脂(TG、TCHO、HDL-C、LDL-C)、血糖及HbA1C差异,并分析老年糖尿病患者阿司匹林AR发生的相关机制。结果经TEG检测:AR比率30.00%(48/160)、ASR比率27.50%(44/160)、AS组比率42.50%(68/160);三组患者血脂(TG、TCHO、HDL-C)组间比较均无显著差异(F=2.356、3.147、5.850、0.948,均P>0.05);AS组和ASR组LDL-C、HbA1C、GLU表达水平(2.20±0.56)mmol/L、(7.47±1.52)%、(7.78±1.33)mmol/L、(2.25±0.61)mmol/L、(7.61±1.23)%、(8.14±1.47)mmol/L与AR组比较结果均具有明显差异(F=9.948、8.129、7.334,均P<0.05或P<0.01);经Logistic分析提示:LDL-C、HbA1C表达与老年糖尿病患者AR发生的关系密切(P<0.05,OR=3.97,95%CI=0.889~9.714)、(P<0.01,OR=3.11,95%CI=2.42~4.01)。结论LDL-C、HbA1C表达均为老年糖尿病患者阿司匹林AR发生的独立危险因素,血糖控制不佳者更易出现AR。
