长春西汀辅助治疗老年冠心病合并2型糖尿病患者的临床疗效及对ICAM-1的影响

Clinical Efficacy of Vinpocetine in Adjunctive Therapy of Elderly Patients with Coronary Heart Disease Associated by Type 2 Diabetes Mellitus and Its Effect on ICAM-1

目的 探讨长春西汀辅助治疗老年冠心病合并2型糖尿病患者的临床疗效及对细胞间黏附分子-1(ICAM-1)的影响。方法 对2016年1月—2017年1月收治的老年冠心病合并2型糖尿病患者114例的临床资料进行回顾性分析,根据治疗方法分为观察组60例和对照组54例,对照组予以常规治疗,观察组在对照组治疗基础上联合长春西汀治疗。治疗结束后比较2组血脂水平、外周血炎性因子水平、心血管相关指标及心血管事件和不良反应发生情况。结果 治疗后,观察组高密度脂蛋白胆固醇(HDL-C)、一氧化氮(NO)、左心室射血分数(LVEF)水平高于对照组和治疗前,总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、白介素-6(IL-6)、C反应蛋白(CRP)、内皮素(ET)、ICAM-1水平低于对照组和治疗前( P <0.05, P <0.01)。观察组心血管事件发生率低于对照组( P <0.05)。2组均未发生不良反应。结论 长春西汀辅助治疗老年冠心病合并2型糖尿病具有良好的临床效果,对ICAM-1水平亦存在改善作用,且具有一定的安全性。

Objective To investigate clinical efficacy of Vinpocetine in adjunctive therapy of elderly patients with coronary heart disease associated by type2 diabetes mellitus and its effect on intercellular adhesion molecule-1 (ICAM-1). Methods Clinical data of114 elderly patients with coronary heart disease associated by type2 diabetes mellitus admitted during January2016 and January2017 was retrospectively analyzed, and the patients were divided into control group ( n =54) and observation group ( n =60) according to different therapeutic methods. Control group was treated with conventional treatments, while observation group was added with Vinpocetine on the basis of treatment for control group. After treatment, levels of blood lipid, and inflammatory factors of peripheral blood, indexes of cardiovascular function, and incidence rates of cardiovascular events and adverse reactions were compared between two groups. Results After treatment in observation group, levels of high density lipoprotein cholesferol (HDL-C), nitric oxide (NO) and left ventricular ejection fraction (LVEF) were significantly higher, while levels of total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), interleukin-6 (IL-6), C-reactive protein (CRP), endothelin (ET) and ICAM-1 were significantly lower than those in control group and before treatment in the same group ( P <0.05, P <0.01). Incidence rate of cardiovascular events in observation group was significantly lower than that in control group ( P <0.05). No adverse reactions were found in two groups. Conclusion Vinpocetine has good clinical efficacy in adjunctive therapy of elderly patients with coronary heart disease associated by type2 diabetes mellitus, and it may also improve ICAM-1 level with a certain safety.
