进展性缺血性脑卒中发生的相关危险因素分析

Risk Factors Associated with Pathogenesy of Progressive Cerebral Ischemic Stroke

目的 探讨进展性缺血性脑卒中(PIS)发生的危险因素。方法 对2016年10月—2018年8月收治的急性缺血性脑卒中169例的临床资料进行回顾性分析。将美国国立卫生研究院卒中量表(NIHSS)评分增加≥3分的58例作为PIS组,其余111例非PIS患者作为对照组。对比2组一般资料,记录入院时生命体征情况;检测2组甘油三酯、总胆固醇、低密度脂蛋白胆固醇、同型半胱氨酸、尿酸、空腹血糖;对比2组脑梗死部位,对危险因素进行多因素Logistic回归分析。结果 糖尿病史、高血压病史、空腹血糖升高及入院体温升高是PIS发生的独立危险因素(P<0.05)。结论 在临床工作中,对脑梗死急性期患者要注意控制和严密观察血糖、血压、体温情况,以改善预后。

Objective To investigate risk factors associated with pathogenesy of progressive cerebreal ischemic stroke (PCIS). Methods Clinical data of169 patients with acute cerebral ischemic stroke admitted during October2016 and August2018 was retrospectively analyzed. A total of 58 patients with national institution of health stroke scale (NIHSS) score increased equal or more than3 points were selected as PCIS group, and other111 non-PCIS patients were used as control group. In two groups, general data was compared, and conditions of vital signs at the time of admission were recorded;levels of triglyceride, total cholesterol, low density lipoprotein cholestorol, homocysteine, uric acid and fasting blood glucose were detected;location of cerebral infarction was compared between the two groups, and risk factors were analyzed by logistic regression. Results Diabetes history, history of hypertension, elevated level of fasting blood sugar and elevated body temperature were independent risk factors for pathogenesy of progressive cerebreal ischemic stroke ( P <0.05). Conclusion It is suggested that attention should be paid to controlling and closely observing blood sugar, blood pressure and body temperature in patients with acute cerebral infarction in clinic so as to improve prognosis.
