补阳还五汤加味治疗气虚血瘀型中风后遗症疗效刍议

Therapeutic Effect of Buyang Huanwu Decoction on Stagnation of Qi Deficiency and Blood Stasis

目的分析针对气虚血瘀型中风后遗症患者应用中医药补阳还五汤加味方治疗的临床效果。方法选取门诊自2015年1月-2018年12月止诊治的76例气虚血瘀型中风后遗症患者为回顾分析对象,数字法来分组为观察组、对照组。对照组患者采取常规对症治疗方案,观察组患者在对照组常规治疗基础上联合应用中医药补阳还五汤加味治疗。结果观察组患者治疗总有效率高达94.74%,高于同期对照组的81.58%(χ^2=4.668,P=0.000);两组患者治疗之前全血黏度、血浆黏度以及纤维蛋白原指标均较高(t=0.356,0.734,0.619;P=0.092,0.065,0.103);治疗之后观察组患者的全血黏度、血浆黏度和纤维蛋白原指标均显著低于对照组患者(t=4.628,3.928,4.153;P=0.000,0.001,0.000);两组患者治疗之前ADL、NIHSS以及中医证候积分均较高(t=0.403,0.617,0.802;P=0.079,0.093,0.152);治疗之后观察组患者的ADL、NIHSS以及中医证候积分均显著低于对照组患者(t=5.038,4.938,6.203;P=0.000,0.001,0.000)。结论针对气虚血瘀型中风后遗症患者应用中医药补阳还五汤加味进行治疗可取得满意效果,有助于改善患者临床症状和临床指标,有助于促进患者康复,可改善患者生活自理能力,该药物治疗方案值得临床应用和推广。

Objective To analyze the clinical effect of traditional Chinese medicine Buyang Huanwu Decoction on patients with qi stagnation and blood stasis type sequelae. Methods 76 patients with qi stagnation and blood stasis type sequelae from January 2015 to December 2018 were selected as retrospective analysis subjects. The digital method was divided into observation group and control group. The patients in the control group were treated with conventional symptomatic treatment. The patients in the observation group were treated with traditional Chinese medicine Buyang Huanwu Decoction on the basis of conventional treatment in the control group.Results The total effective rate of the observation group was 94.74%, which was higher than that of the control group 81.58%(χ^2=4.668, P=0.000). The blood glucose, plasma viscosity and fibrinogen index were higher before treatment.(t=0.356, 0.734, 0.619;P=0.092, 0.065, 0.103);the whole blood viscosity, plasma viscosity and fibrinogen index of the observation group were significantly lower than those of the control group after treatment(t =4.628, 3.928, 4.153, P =0.000, 0.001, 0.000);ADL, NIHSS, and TCM syndrome scores were higher in both groups before treatment(t=0.403, 0.617, 0.802;P=0.079, 0.093, 0.152);ADL, NIHSS, and TCM syndrome scores were significantly lower than those in the control group(t=5.038, 4.938, 6.203;P=0.000, 0.001, 0.000). Conclusion The treatment of qi stagnation and blood stasis type sequelae patients with traditional Chinese medicine Buyang Huanwu Decoction can achieve satisfactory results, help to improve clinical symptoms and clinical indicators, help to promote patient recovery, and improve patients’ self-care ability. The drug treatment plan is worthy of clinical application and promotion.
