糖尿病视网膜病变患者疾病感知现状及影响因素分析

Analysis on the status of disease perception and influencing factors in patients with diabetic retinopathy

目的:分析糖尿病视网膜病变(DR)患者的疾病感知现状及其影响因素,为提高临床诊疗水平提供理论参考。方法:抽取2015年1月~2018年10月在安徽省某眼科医院就诊的DR患者208例。收集患者基本信息,采集焦虑自评量表(SAS)、抑郁自评量表(SDS)和Broadbent简易版疾病感知问卷(BIPQ)数据,并对结果进行分析。结果:患者在居住地、文化程度、糖尿病病程、DR类型、视力情况等各因素的BIPQ总分上差异有统计学意义(P<0.05),患者在焦虑、抑郁等情绪得分与BIPQ总分存在一定的相关性(R=0.322和0.511,P<0.05)。多元线性回归分析显示,影响DR患者疾病感知得分的因素按照贡献大小排序依次为抑郁、焦虑和视力。结论:视力状况差以及焦虑、抑郁的患者对疾病感知的影响较为显著。

Objective: To investigate the status of illness perception and its influencing factors in patients with diabetic retinopathy(DR) for theoretical evidence to improve the diagnosis and treatment. Methods: Basic information was obtained from 208 DR patients treated in an ophthalmic hospital in Anhui province from January 2015 to October 2018 using self-rating anxiety scale(SAS),self-rating depression scale(SDS) and simplified version brief illness perception questionnaire(BIPQ),and the results were analyzed. Results: The difference was significant in BIPQ scores by place of residence,educational levels,DR course,type of DR and visual acuity as well as other factors( P< 0.05).Patients′ emotional scores such as anxiety and depression were correlated with the total score of BIPQ( R =0.322 and 0.511,respectively, P< 0.05).Multivariate analysis indicated that the factors affecting disease perception were associated successively with depression,anxiety and visual acuity. Conclusion: Disease perception in DR patients is involved in poor vision,anxiety and depression.
