摘要 目的:分析磁共振波谱(MRS)对老年难治性癫痫海马病变的诊断意义.方法:对100例老年难治性癫痫(观察组)、100例健康体检正常人员(对照组)行磁共振成像(MRI)及MRS检查.结果:对照组MRI、MRS检查均正常;观察组100例患者MRS检查海马异常率100.00%显著高于MRI的75.00%(P<0.05);观察组NAA/Cr、NAA/Cho、NAA/(Cho+Cr)比值分别为(1.02±0.17)、(1.04±0.25)、(0.50±0.07),均显著低于对照组的(1.46±0.12)、(1.47±0.16)、(0.77±0.08),差异有统计学意义(P<0.05);观察组病侧NAA/Cr、NAA/Cho、NAA/(Cho+Cr)比值均显著低于对侧(P<0.05).结论:MRS相比MRI对老年难治性癫痫患者海马病变的检出率显著高,在难治性癫痫诊断有重要指导价值. Objective:To analyze the diagnostic value of magnetic resonance spectroscopy(MRS)in elderly intractable epilepsy with hippocampal lesions.Methods:The magnetic resonance imaging(MRI)and MRS were performed in 100 elderly patients with intractable epilepsy(observation group)and 100 normal controls(control group).Results:Results of MRI and MRS in the control group were normal.The observation group(100 cases)the abnormal rate of hippocampus examined by MRS was 100.00%,significantly higher than that by MRI(75.00%)(P < 0.05).Ratios of NAA/Cr,NAA/Cho and NAA/(Cho+Cr)in the observation group were significantly lower than those in the control group[(1.02±0.17),(1.04±0.25)and (0.50±0.07)vs(1.46±0.12),(1.47±0.16)and (0.77±0.08)](P < 0.05).Ratios of NAA/Cr,NAA/Cho and NAA/(Cho+Cr)of the ipsilateral side were significantly lower than those of the contralateral side in the observation group(P < 0.05).Conclusion:Compared with MRI,the detection rate of hippocampal lesions by MRS in elderly patients with intractable epilepsy is significantly higher,and it is of great guidance value in the diagnosis of intractable epilepsy.
