摘要
目的分析尿微量白蛋白、尿转铁蛋白、尿NAG和尿α1-MG联合检测在2型糖尿病早期肾损害诊断中的应用价值。方法选取2016年9月至2018年10月我院收治的2型糖尿病患者89例,均行尿微量白蛋白、尿转铁蛋白、尿NAG和尿α1-MG检测,并以肾穿刺活检结果为标准,统计联合检测的诊断价值,并比较糖尿病及糖尿病肾损害者尿微量白蛋白、尿转铁蛋白、尿NAG和尿α1-MG水平。结果经肾穿刺活检发现, 89例2型糖尿病患者有41例肾损害。尿微量白蛋白、尿转铁蛋白、尿NAG和尿α1-MG联合检测2型糖尿病肾损害的准确性为87.64%,敏感性为87.80%,特异性为87.50%;2型糖尿病肾损害者的尿微量白蛋白、尿转铁蛋白、尿NAG及尿α1-MG水平均高于2型糖尿病者(P <0.05)。结论尿微量白蛋白、尿转铁蛋白、尿NAG和尿α1-MG联合检测在2型糖尿病早期肾损害诊断中效果显著,可发现早期肾损害。
Objective To analyze the application value of combined detection of urinary microalbumin, urinary transferrin, urinary NAG and α1-MG in the diagnosis of early renal damage in type 2 diabetes mellitus. Methods 89 cases of patients with type 2 diabetes mellitus admitted to our hospital from September 2016 to October 2018 were given detection of urine microalbuminuria, transferrin, urinary NAG and α1-MG. Taking the results of renal biopsy as the standard, the diagnostic value of combined detection was recorded, and the urinary microalbumin, urinary transferrin, urinary NAG and urinary α1-MG levels were compared among patients with type 2 diabetes mellitus or early renal damage in type 2 diabetes mellitus. Results 41 cases of type 2 diabetes mellitus with renal damage were found by renal biopsy. The accuracy, sensitivity and specificity of combined detection of urinary microalbuminuria, urinary transferrin, urinary NAG and α1-MG were respectively 87.64%, 87.80% and 87.50%. The urinary microalbumin, urinary transferrin, urinary NAG and α1-MG levels in type 2 diabetic patients with renal damage were significantly higher than those in type 2 diabetic patients(P <0.05). Conclusions The combined detection of urinary microalbumin, urinary transferrin, urinary NAG and urinary α1-MG in the diagnosis of early renal damage in type 2 diabetes has significant effect, and the early renal damage can be found.
