先兆早产患者产前抑郁的影响因素调查

Investigation on Influencing Factors of Prenatal Depression in Patients with Threatened Premature Delivery

目的分析先兆早产患者产前抑郁的影响因素。方法回顾性分析2016年8月至2018年8月我院收治的70例先兆早产患者的临床资料,根据EPDS评分分为抑郁组(EPDS>9.5分, 27例)与非抑郁组(EPDS≤9.5分, 43例),分析先兆早产患者产前抑郁的影响因素。结果抑郁组孕周<34周、有不良孕产史、高中及以下文化程度、自费、家庭人均月收入<5 000元、唾液皮质醇含量≥11.50 nmol/L的比例均高于非抑郁组(P <0.05)。Logistic分析显示,年龄、孕周、不良孕产史、医疗费用支出形式、家庭人均月收入、唾液皮质醇含量是导致先兆早产患者产前抑郁的相关危险因素(OR>1, P <0.05);文化程度是其保护因素(OR<1, P <0.05)。结论为缓解先兆早产患者产前抑郁情绪,应根据其相关影响因素实施针对性干预,以改善妊娠结局。

Objective To analyze the influencing factors of prenatal depression in patients with threatened premature delivery.Methods The clinical data of 70 cases of patients with threatened premature delivery admitted to our hospital from August 2016 to August2018 were retrospectively analyzed. According to the EPDS score, they were divided into depression group(EPDS>9.5 points, 27 cases) and non-depression group(EPDS≤9.5 points, 43 cases). The influencing factors of prenatal depression in patients with threatened premature delivery were analyzed. Results The rates of gestational weeks <34 weeks, adverse pregnancy and delivery history, high school and below education level, self expenditure, family per capita monthly income <5 000 yuan, and salivary cortisol content ≥11.50 nmol/L of the depression group were higher than those of the non-depression group(P <0.05). Logistic analysis showed that the age, gestational weeks,adverse pregnancy and delivery history, medical expenditure form, family per capita monthly income, and salivary cortisol content were risk factors that lead to the prenatal depression in patients with threatened premature delivery(OR>1, P <0.05), and the education level was the protective factor(OR<1, P <0.05). Conclusions In order to alleviate the prenatal depression in patients with threatened premature delivery,targeted intervention should be carried out according to the related influencing factors to improve the pregnancy outcomes.
