摘要
目的 观察磁共振波谱分析(MRS)联合扩散加权成像(DWI)对前列腺癌的诊断准确性,并分析其影响因素。方法 选取2016年3月至2018年12月我院收治的100例前列腺癌患者为研究对象,所有患者均接受MRS和DWI检查,观察MRS联合DWI对前列腺癌诊断的敏感性和特异性,并分析影响诊断准确性的因素。结果 MRS联合DWI对前列腺癌诊断正确率为95.00%,明显高于单独MRS、DWI的80.00%、82.00%(P均<0.05);MRS联合DWI对前列腺癌诊断的敏感性为98.75%,特异性为95.47%,明显高于单独MRS的82.31%、78.59%和单独DWI的84.05%、82.36%(P均<0.05);中低分化、有转移、TNM分期为Ⅲ+Ⅳ期、前列腺特异抗原≥10μg·L^-1、有神经浸润的前列腺癌患者的诊断正确率较高(P均<0.05);将单因素分析有意义的因素作为自变量,将诊断正确率作为因变量进行多因素分析,结果显示分化程度和TNM分期是影响患者诊断正确率的独立因素(P均<0.05)。结论 MRS联合DWI对前列腺癌的诊断正确率较高,且诊断正确率受前列腺癌分化程度和TNM分期的影响。
Objective To observe the accuracy of magnetic resonance spectroscopy (MRS) combined with diffusion weighted imaging (DWI) in the diagnosis of prostate cancer, and to analyze its influencing factors. Methods A total of 100 patients with prostate cancer who received treatment in our hospital from March 2016 to December 2018 were selected. All the patients were examined by MRS and DWI. The sensitivity and specificity of MRS combined with DWI in the diagnosis of prostate cancer were observed, and the factors affected the accuracy of diagnosis were analyzed. Results The correct diagnosis rate of MRS combined with DWI for prostate cancer was 95.00%, and was significantly higher than those of single MRS (80.00%) and DWI (82.00%, P <0.05). The sensitivity of MRS combined with DWI in the diagnosis of prostate cancer was 98.75% and the specificity was 95.47%, which was significantly higher than those of single MRS (82.31% and 78.59%) and DWI (84.05% and 82.36%, P <0.05). The corret diagnosis rate of prostate cancer patients with middle and low differentiation, metastasis, TNM staging Ⅲ+Ⅳ stages, prostate specific antigen > 10 μg·L ^-1 and nerve infiltration were higher ( P < 0.05 ). The degree of differentiation and TNM staging were the factors affecting the corret diagnosis rate of the results of the patient’s examination( P <0.05). Conclusion The corret diagnosis rate of MRS combined with DWI for prostate cancer is higher, and is influenced by differentiation degree and TNM stage.
