经皮椎间孔镜治疗腰椎间盘突出症的临床观察

Clinical Observation on the Treatment of Lumbar Disc Herniation by Percutaneous Intervertebral Foraminoscope

目的探究腰椎间盘突出症患者应用经皮椎间孔镜治疗的效果与临床应用价值.方法选取2017年1月-2018年1月在我院就诊的128例腰椎间盘突出症患者作为研究对象,根据不同的治疗方法分为两组,对照组64例患者应用保守治疗,观察组64例患者应用经皮椎间孔镜治疗,对比两组腰椎间盘突出症患者术前、术后1周、术后3个月、术后1年的疼痛评分、JOA评分与术后腰部功能恢复情况(优良率).结果观察组与对照组患者术前的疼痛评分、JOA评分对比,差异无统计学意义(P>0.05).术后1周、术后3个月、术后1年的疼痛评分、JOA评分均有改善(P<0.05),观察组术后疼痛评分比对照组低,差异有统计学意义(P<0.05).观察组术后的JOA评分比对照组高,差异有统计学意义(P<0.05).随访1年,观察组的腰部功能恢复优良率高于对照组,差异有统计学意义(P<0.05).结论腰椎间盘突出症患者应用经皮椎间孔镜治疗,疗效理想,手术操作简单,患者的疼痛感低,术后恢复情况优良.

Objective To investigate the effect and clinical value of percutaneous intervertebral foraminoscope in the treatment of lumbar disc herniation. Methods From January 2017 and January 2018, a total of 128 patients with lumbar disc herniation in our hospital were divided into two groups according to the different treatment methods, 64 cases in control group were treated with conservative treatment, 64 cases in the observation group were treated with percutaneous intervertebral foraminoscope, preoperative and postoperative pain score, JOA score and the function recovery of postoperative lumbar (the excellent and good rate ) of 1 week, 3 months and 1 year were compared between two groups. Results There was no significant difference in preoperative pain score and JOA score between the observation group and the control group (P > 0.05). The postoperative pain score and JOA score were improved in 1 week, 3 months and 1 year (P < 0.05). The postoperative pain score of the observation group was lower than that of the control group (P < 0.05). The postoperative JOA score of the observation group was higher than that of the control group (P < 0.05). After a one-year follow-up, the recovery rate of lumbar function in the observation group was higher than that in the control group, with significant difference (P < 0.05). Conclusion Percutaneous intervertebral foraminoscope is an effective method for the treatment of lumbar disc herniation. The operation is simple, the patients feel less pain, and the postoperative recovery is better.
