摘要
目的观察阿利沙坦酯对早期糖尿病肾病(DN)患者血清炎症因子、肾功能的影响。方法采用随机对照方法,将65例早期DN患者分为对照组32例及观察组33例。对照组患者给予糖尿病基础治疗。观察组在对照组治疗基础上给予阿利沙坦酯240mg/次,1次/d,晨服。治疗16周后,比较两组治疗前后空腹血糖(FPG)、糖化血红蛋白(HbA1c)、尿微量白蛋白/尿肌酐(UACR)、血肌酐(SCr)、血清胱抑素(CysC)、超敏C反应蛋白(Hs-CRP)、白介素-6(IL-6)水平的变化。结果治疗后,两组FPG、SCr、HbA1c水平差异无显著性(P>0.05),两组患者Hs-CRP、IL-6、UACR、CysC水平较治疗前均明显下降,差异均有统计学意义(P<0.05),且观察组Hs-CRP、IL-6、UACR、CysC水平下降更明显,差异有统计学意义(P<0.05)。结论阿利沙坦酯可以改善早期糖尿病肾病患者炎症状态,降低尿微量白蛋白和血清胱抑素的水平,改善肾功能。
Objective To observe the effect of allisartan isoproxil on serum inflammatory cytokines and renal function in patients with early diabetic nephropathy(DN).Methods 65 patients with early DN were divided into 32 cases of control group and 33 cases of observation group.Patients in the control group were given basic treatment for diabetes.The observation group was given allisartan isoproxil 240mg/time and 1 time/day in the morning after treatment in the control group.After 16 weeks of treatment,changes in fasting blood glucose(FPG),glycosylated hemoglobin(HbA1c),urinary microalbumin/ urinary creatinine(UACR),serum creatinine(SCr),serum cystatin(CysC),hypersensitive c-reactive protein(Hs-CRP)and interleukin-6(IL-6)levels before and after treatment were compared between the two groups.Results After treatment,there was no significant difference in the levels of FPG,SCr and HbA1c between the two groups(P>0.05),while the levels of Hs- CRP,IL-6,UACR and CysC in the two groups were significantly lower than before treatment(P<0.05),and the levels of Hs- CRP,IL-6,UACR and CysC in the observation group were significantly lower(P<0.05).Conclusion Allisartan isoproxil can improve inflammation,decrease urinary microalbumin and serum cystatin and improve renal function in patients with early diabetic nephropathy.
