麻杏石甘汤联合阿奇霉素治疗儿童支原体肺炎的疗效及对血清炎性因子的影响 被引量:11

Efficacy of Maxingshigan Decoction Combined with Azithromycin in Treatment of Mycoplasma Pneumonia in Children and its Effect on Serum Inflammatory Factors

摘要 目的:探讨麻杏石甘汤联合阿奇霉素治疗儿童支原体肺炎的疗效及对血清炎性因子的影响.方法:选取2017—2018年芜湖市中医医院儿科收治的支原体肺炎患儿80例,采用随机数字表法分为对照组和观察组,每组40例.对照组患儿采用阿奇霉素治疗,观察组患儿采用麻杏石甘汤联合阿奇霉素治疗.比较两组患儿治疗前后临床症状改善情况,C反应蛋白(CRP)、乳酸脱氢酶(LDH)水平,临床疗效,不良反应发生情况及复发情况的差异.结果:观察组患儿咳嗽痰鸣消失时间、干湿性啰音消失时间及体温恢复时间明显短于对照组;治疗后,两组患儿CRP、LDH水平较治疗前明显降低,且观察组患儿明显低于对照组;观察组患儿的总有效率为95.00%(38/40),明显高于对照组的77.50%(31/40),上述差异均有统计学意义(P<0.05).观察组、对照组患儿的不良反应发生率分别为2.50%(1/40)、5.00%(2/40),差异无统计学意义(P>0.05);随访6个月,观察组患儿的复发率为5.00%(2/40),明显低于对照组的20.00%(8/40),差异有统计学意义(P<0.05).结论:麻杏石甘汤联合阿奇霉素治疗儿童支原体肺炎,可显著减轻炎症反应,改善肺功能,效果显著,复发率低. OBJECTIVE:To probe into the efficacy of Maxingshigan decoction combined with azithromycin in treatment of mycoplasma pneumonia in children and its effect on serum inflammatory factors.METHODS:80 children with mycoplasma pneumonia admitted into pediatric department of Wuhu Hospital of Traditional Chinese Medicine from 2017 to 2018 were selected and divided into control group and observation group via random number table,with 40 cases in each group.The control group was treated with azithromycin,while the observation group was given Maxingshigan decoction combined with azithromycin.Differences in improvements of clinical symptoms,C-reactive protein(CRP),lactic dehydrogenase(LDH)levels,clinical efficacy,incidences of adverse drug reactions and recurrence rate between two groups were compared.RESULTS:The disappearance time of expectoration,disappearance time of rale and recovery time of temperature of observation group were significantly shorter than those of the control group;after treatment,the CRP and LDH levels of both groups had been significantly decreased,and those of the observation group were significantly lower than the control group;the total effective rate of observation group was 95.00%(38/40),which was significantly higher than that of the control group(77.50%,31/40),with statistically significant difference(P<0.05).The incidences of adverse drug reactions of observation group and control group were respectively 2.50%(1/40)and 5.00%(2/40),the difference had no statistical significance(P>0.05);after 6-month follow-up,the recurrence rate of observation group was 5.00%(2/40),which was significantly lower than that of the control group(20.00%,8/40),with statistically significant difference(P<0.05).CONCLUSIONS:Maxingshigan decoction combined with azithromycin in treatment of mycoplasma pneumonia in children can effectively relief physical inflammatory reaction,improve pulmonary function,with remarkable effects and low recurrence rate.
