颅脑磁共振成像在帕金森病与血管性帕金森综合征鉴别诊断中的价值分析 被引量:2

Analysis on the Value of Craniocerebral Magnetic Resonance Imaging in Differential Diagnosis of Parkinson’s Disease and Vascular Parkinsonism

摘要 目的探讨颅脑磁共振成像(MRI)鉴别诊断帕金森病(PD)、血管性帕金森综合征(VP)的价值。方法选取PD患者37例(PD组)、VP患者33例(VP组)与健康人群35例(对照组)为研究对象,均予以颅脑MRI检查,比较三组的黑质致密带宽度(WPCSN)、中脑直径以及WPCSN/中脑直径,分析颅脑MRI对PD组、VP组各项颅脑病变的检出率。结果三组的中脑直径比较无显著差异(P>0.05);PD组的WPCSN、WPCSN/中脑直径均显著低于其他两组(P<0.05),而VP组与对照组比较无显著差异(P>0.05)。PD组的基底节病变、皮层下白质病变、下丘脑病变及脑干损伤检出率显著低于VP组(P<0.05)。结论颅脑MRI对PD、VP具有良好的鉴别诊断价值。 Objective To investigate the value of craniocerebral magnetic resonance imaging(MRI) in differential diagnosis of Parkinson’s disease(PD) and vascular Parkinsonism(VP). Methods 37 cases of PD patients(PD group), 33 cases of VP patients(VP group)and 35 cases of healthy people(control group) were selected as research objects. All cases received craniocerebral MRI examination, and the width of pars compacta of substantia nigra(WPCSN), midbrain diameter and WPCSN/midbrain diameter were compared among three groups. The detection rates of various brain lesions of PD group and VP group by craniocerebral MRI were analyzed. Results No significant difference was found in the midbrain diameter among the three groups(P >0.05). The WPCSN and WPCSN/midbrain diameter of PD group were significantly lower than those of VP group and control group(P <0.05), while no significant difference was found between VP group and control group in the WPCSN and WPCSN/midbrain diameter(P >0.05). The detection rates of basal ganglia lesion, subcortical white matter lesion, hypothalamic lesion and brain stem injury in the PD group were significantly lower than those in the VP group(P <0.05).Conclusions Craniocerebral MRI has good value in the differential diagnosis of PD and VP.
