某院1039例临床药师会诊分析及药学会诊模式探讨

Analysis of 1 039 cases of clinical pharmacist consultations and exploration of consultation mode

目的:通过某院临床药师会诊的形式和效果,探讨药学会诊的工作模式。方法:对该院2017年1月-2018年9月临床药师会诊记录共1 039例进行统计分析。结果:该院药学会诊例次呈增长趋势,从2017年第1季度的63例增加至2018年第3季度的225例;单学科会诊中会诊类别以抗感染为主,共778例(79.6%);主要药学会诊意见为调整药物品种,共762例(73.3%);药学会诊意见被临床采纳的有984例(94.7%),会诊72 h后治疗有效828例(79.7%)。结论:临床药师通过多学科药学会诊、制定不同专业药学会诊路径等新会诊模式的探索,提高药学会诊质量,保障临床合理用药。

OBJECTIVE To explore the the working mode of pharmaceutical consultation through the form and effect of clinical pharmacist consultations. METHODS 1 039 cases of clinical pharmacists’ consultation records from January 2017 to September 2018 were statistically analyzed. RESULTS The number of pharmaceutical consultation cases showed an increasing trend, from 63 cases in the first quarter of 2017 to 225 cases in the third quarter of 2018. Anti-infection was the main type of consultation with a total of 778 cases(79.6%). Main pharmaceutical consultation opinion was adjustment of drug varieties in 762 cases(73.3%). The opinions of pharmaceutical consultation were adopted in 984 cases(94.7%), and 828 cases(79.7%) were effectively treated 72 h after consultation. CONCLUSION Clinical pharmacists play an active role in improving the quality of pharmaceutical consultation and ensuring rational clinical medication by exploring new consultation modes such as multi-disciplinary pharmaceutical consultation and formulating different professional pharmaceutical consultation paths.
