基于聚类分析的中医药治疗偏头痛用药规律探讨

Study on the Law of Traditional Chinese Medicine on Migraine and Related Problems

目的观察中医药治疗偏头痛的常用药物和配伍规律。方法以1989年1月至2018年3月在省级以上期刊发表,以中药汤剂治疗偏头痛,且经过临床疗效观察有效的中药复方为研究对象,对其用药进行聚类分析,探索中医药治疗偏头痛的常用药物和配伍规律。结果偏头痛使用最多的药物为川芎、白芷、甘草、白芍、当归、天麻、细辛、柴胡、全蝎、僵蚕、钩藤、蔓荆子。聚类分析显示,聚5类药物稳定,偏头痛常用药物可分为4类,包括平肝潜阳,活血通络,补益气血,祛风止痛等药物。结论偏头痛的病位在脑,与五脏均有关系,但与肝、脾关系最为密切,在偏头痛发病过程中以肝风内动、痰瘀阻络为主要病机。

Objective:To explore the common drugs and compatibility rules of Chinese medicine on migraine.Methods:The effective Chinese herbal compound prescriptions to treat migraine,published in journals at or above the provincial level from January 1989 to March 2018,were observed.Cluster analysis was carried out to explore the common drugs and compatibility rules of Chinese medicine on migraine.Results:The most commonly used drugs for migraine were Rhizome of Chuanxiong,dahurica,liquorice,radices paeoniae alba,Radix Angelicae,Gastrodia elata,Asarum,Bupleurum Bupleurum,Scorpio,Silkworm,Rhynchophylla and Thorns.Cluster analysis showed that the commonly used drugs for migraine could be divided into 4 categories,including drugs for smoothing liver and latenting Yang,activating blood and dredging collaterals,replenishing Qi and blood,dispelling wind and relieving pain.Conclusion:The position of migraine in the brain is related to the five zang organs,but it is most closely related to liver and spleen.In the process of migraine,the main pathogenesis is liver wind movement and phlegm stasis blocking collaterals.
