中医理筋手法加中药外敷治疗急性踝关节扭伤的临床研究

Clinical Observation on Treatment of Acute Ankle Sprain with Massage Tendons and Chinese Medicine External Application

目的观察中医理筋手法加中药外敷治疗急性踝关节扭伤的临床疗效。方法将80例患者按随机数字表法分为两组,对照组仅接受石膏外固定治疗,观察组在接受中医理筋手法加中药肿意膏、活血膏外敷,于首诊时、首诊后第7天、第14天、第28天进行视觉模拟量表(VAS)评分并测量患足肿胀体积,于首诊时及首诊后3个月进行美国矫形外科足踝协会(AOFAS)评分。结果两组组内比较,对照组首诊后第14天、第28天VAS评分及肿胀体积较首诊日明显降低(P<0.05),观察组首诊后第7天、第14天、第28天VAS评分及肿胀体积较首诊日明显降低(P<0.05),首诊后3个月两组患者AOFAS评分较首诊日明显增高(P<0.05);两组组间比较,观察组第7天VAS评分及肿胀体积明显低于对照组(P<0.05);首诊后3个月两组患者AOFAS评分比较,观察组高于对照组(P<0.05)。结论中医理筋手法加中药外敷在治疗急性踝关节扭伤方面有较好疗效,能较快缓解患肢疼痛及肿胀症状,改善踝关节远期功能。

Objective:To observe the clinical effects of massage tendons combined with Chinese medicine external application on acute ankle sprains.Methods:80 patients were randomly divided into two groups,40 cases in each.The control group received external fixation with gypsum,and the treatment group received massage tendons combined with Chinese medicine external application.VAS was scored at the first visit,7 days,14 days and28 days after it,and the swelling volume of the affected foot was measured.AOFAS was scored 3 months after the first visit.Results:In the control group,VAS score and foot swelling volume were significantly decreased on the14 th and 28 th day after the first visit(P<0.05);those of the experimental group were significantly decreased on the 7 th,14 th and 28 th day after the first visit(P<0.05);VAS scores and foot swelling volume of the experimental group on day 7 were significantly lower than those of the control group(P<0.05);3 months after the first treatment,AOFAS scores in the experimental group were higher than those in the control group(P<0.05).Conclusion:Massage tendons combined with Chinese medicine external application has a good curative effect on acute ankle sprain.It can quickly relieve the pain and swelling symptoms of affected limbs and improve the long-term function of ankle joint.
