可视喉镜与纤维支气管镜在困难气管插管中的应用比较

Exploration on comparison of the application of video laryngoscope and fiberoptic bronchoscope in difficult tracheal intubation

目的探讨可视喉镜与纤维支气管镜在困难气管插管中的应用比较。方法选取我院急救科2016年4月~2018年3月收纳治疗的抢救患者62例作为研究对象,采用随机数字表法将其分为可视喉镜组以及纤维支气管镜组,每组31例。观察分析两组患者在不同插管方式下的一次插管成功率、插管时间、血压、心率以及并发症发生率等。结果两组患者在一次插管成功率、插管时间以及并发症发生率方面,差异无统计学意义(P>0.05);插管前两组患者心率、舒张压、收缩压指标比较差异无统计学意义(P>0.05),插管后两组患者心率、舒张压、收缩压指标比较差异有统计学意义(P<0.05)。结论对于实施抢救且需进行困难气管插管的患者实施两种插管后均具有较高安全性,而纤维支气管镜在口咽喉的黏膜损伤相对较小,选择插管方式应根据患者情况合理应用。

Objective To explore the comparison of the application of video laryngoscope and fiberoptic bronchoscope in difficult tracheal intubation. Methods 62 rescue patients who were admitted and treated in department of emergency in our hospital from April 2016 to March 2018 were selected as research objects.They were divided into the video laryngoscope group and the fiberoptic bronchoscope group according to the random number method,with 31 cases in each group.The success rate of intubation on the first attempt,intubation time,blood pressure,heart rate and complication rate of patients in two groups under different intubation methods were observed and analyzed. Results There was no statistically significant difference in the success rate of intubation on the first attempt,intubation time and incidence of complications between the two groups (P > 0.05).There was no difference in heart rate,diastolic blood pressure and systolic blood pressure between the two groups before intubation (P > 0.05). There were significant differences in heart rate,diastolic blood pressure and systolic blood pressure between the two groups after intubation (P < 0.05). Conclusion For patients with rescue and need for difficult tracheal intubation, both types of intubation have higher safety.The fiberoptic bronchoscope has relatively small mucosal damage in the oropharynx.The choice of intubation should be rationally applied according to the patient's condition.
