多学科协作早期运动预防ICU机械通气患者获得性衰弱的临床观察

Observation of the effect of Multi-disciplinary team for early exercise on the prevention of ICU-acquired weakness in patients with mechanical ventilation

目的:探讨多学科协作早期运动对预防ICU机械通气患者获得性衰弱的影响。方法:将100例机械通气患者按照随机数字表分成观察组和对照组各50例,所有患者遵循国际认可的机械通气患者早期四步运动锻炼方案。对照组采取传统医护合作模式;观察组患者由多学科协作早期运动治疗小组成员对患者进行康复运动指导及监督。对两组干预后的相关指标进行比较。结果:观察组患者开始下床时间、机械通气时间、住ICU时间均少于对照组,差异有统计学意义(P<0.05);观察组患者ICU获得性衰弱、VAP和谵妄发生率均低于对照组,差异有统计学意义(P<0.05)。结论:多学科协作可以保证机械通气患者早期运动的有效实施,促进患者的康复,降低各种并发症的发生。

Objective: To investigate the effect of multi-disciplinary team(MDT) for early exercise on the prevention of ICU-acquired weakness(ICU-AW) in patients with mechanical ventilation.Methods: 100 cases were randomly divided into control group (n=50) and experimental group (n=50).All the patients did early four steps exercise scheme according to the international accepted mechanical ventilation standards, the cases in control group adopted the traditional medical mode of cooperation;while the patients in experimental group accepted the multidisciplinary collaboration.The related indexes of the two groups were compared after interventions finally.Results: The time of getting out of bed, the mechanical ventilation time and ICU staying time were shorten in experimental group, than those in control group;the rehabilitation effect was superior(P<0.05);Besides, the incidences of various complications such as ICU-AW, VAP and delirium were lower than that of the control group (P<0.05).Conclusion: MDT can ensure the effective implementation of early exercise in patients with mechanical ventilation, promote the rehabilitation of patients, reduce the incidence of various complications, meanwhile it is worthy of promotion and application in clinic.
