DSA引导下选择性脊神经根阻滞治疗腰椎间盘突出症疗效及对脊柱稳定性的影响

Effect of DSA Guided Selective Spinal Nerve Root Block in the Treatment of Lumbar Disc Herniation and its Effect on Spinal Stability

目的:观察DSA引导下选择性脊神经根阻滞(SNRB)治疗腰间盘突出的疗效,分析其对脊柱稳定性的影响。方法:以60例L4-S1腰椎间盘突出症患者为A组,给予DSA引导下SNBR治疗,另选取60例作为B组,给予椎旁神经阻滞(PVB)治疗,观察治疗效果。结果:A组治疗总有效率显著高于B组(P<0.05);A组LL、SS、SVA治疗前、治疗后与B组基本相同,且两组治疗后与治疗前也基本相同,差异不显著(P>0.05)。结论:腰椎间盘突出症治疗时,行DSA引导下SNBR治疗可提升治疗效果,且不会明显的影响患者脊柱的稳定性,安全有效。

Objective:To observe the efficacy of DSA guided selective spinal nerve root block(SNRB)in the treatment of lumbar disc herniation and to analyze its effect on spinal stability.Methods:60 cases of L4-S1 lumbar intervertebral disc herniation patients were selected as group A,with treatment of SNBR under the guidance of DSA,and 60 cases of L4-S1 lumbar intervertebral disc herniation patients were selected as group B,with treatment of paravertebral nerve block(PVB),their therapeutic effect were observed.Results:The total effective rate of treatment in group A was significantly higher than that in group B(P<0.05),and LL,SS,SVA before and after treatment in group A were basically the same as those in group B,and the difference after treatment was not significant(P>0.05).Conclusion:In the treatment of protrusion of lumbar intervertebral disc,SNBR therapy under DSA guidance can improve the therapeutic effect,and it will not significantly affect the stability of the spinal column,which is safe and effective.
