瓜蒌薤白半夏汤加味联用沙美特罗替卡松治疗慢性阻塞性肺疾病

Gualuoxiebaibanxia Decoction Combined with Seretide in the Treatment of Chronic Obstructive Pulmonary Disease

目的观察瓜蒌薤白半夏汤加味联用沙美特罗替卡松治疗慢性阻塞性肺疾病(COPD)的效果。方法选取我院治疗的COPD患者132例,随机分为对照组和研究组(各66例)。对照组给予沙美特罗替卡松粉吸入剂,研究组在此基础上服用瓜蒌薤白半夏汤加味。比较两组:总有效率及不良反应;治疗2个疗程后的第1秒用力呼气容积占预计值百分比(FEV1%)、FEV1占用力肺活量(FVC)百分比[(FEV1/FVC)%]、6min步行距离(6MWD)等肺功能指标;动脉血pH值、二氧化碳分压(PaCO2)、氧分压(PaO2)等血气指标。结果研究组总有效率(97.0%)明显高于对照组(81.8%);治疗后,研究组的FEVl%、FEVl/FVC、6MWD、pH、PaO2均显著高于对照组,PaCO2显著低于对照组(均P<0.05);两组不良反应发生率差异无统计学意义。结论瓜蒌薤白半夏汤加味联用沙美特罗替卡松治疗COPD的效果较单用沙美特罗替卡松好,可快速缓解患者症状,改善肺功能及血气指标,且较安全。

Objective To study effects of Gualuoxiebaibanxia decoction combined with seretide in treating chronic obstructive pulmonary disease ( COPD). Methods 132 COPD patients were selected and randomly divided into control group and observation group with 66 cases in each group. Patients in control group were given seretide powder inhalation, and patients in observation group were given Gualuoxiebaibanxia decoction combined with seretide. Clinical efficacy, adverse reactions, FEV1 %, FEV1/FVC ,6 min walking distance( 6MWD), arterial blood pH , partial pressure of carbon dioxide( PaCO2) and oxygen partial pressure( PaO2) were compared. Results The total effective rate in observation group (97. 0%) was significantly higher than that in control group ( 81. 8%). After treatment, FEV1%, FEV1/FVC ,6MWD, pH and PaO2 in observation group were significantly higher than those in control group, while PaCO2 in observation group was significantly lower than that in control group ( P < 0. 05 ). There was no significant difference in the incidence of adverse reactions between the two groups. Conclusion The efficacy of Gualuoxiebaibanxia decoction combined with seretide is better than only seretide in treating COPD, which can relieve clinical symptoms rapidly, improve lung function and blood gas indexes safely.
