Surveillance of food-borne Shigella antibiotic resistance in Jiangsu province from 2015 to 2017

摘要 目的了解2015—2017年江苏省食源性疾病监测分离志贺菌耐药情况。方法按监测方案,根据美国临床实验室标准化研究所(CLSI)推荐的药敏试验抗生素选择原则,采用微量肉汤稀释法,对从腹泻病人粪便或肛门拭子中分离出的119株志贺菌进行15种常见抗生素的药敏鉴定。结果2015-2017年,共复苏、复核鉴定志贺菌119株,耐药率均为100.0%。对不同抗生素耐药率,以红霉素最高(95.8%),其次是萘啶酸(90.8%)、氨苄西林(87.4%)、四环素(76.5%);多重耐药109株,多重耐药率为91.6%,最多1株可耐受9类抗生素。不同年份中四环素、头孢西丁、氨苄西林/舒巴坦、红霉素耐药率差异均有统计学意义(P值均<0.05)。结论江苏省食源性疾病致病菌志贺菌耐药情况较为严重,应进一步加强监测,及时了解致病菌耐药情况,指导临床合理用药。 Objective To understand the drug resistance status of Shigella isolated strains from food-borne disease surveillance in Jiangsu province from 2015 to 2017.Methods According to the surveillance plan and the principle of antibiotic selection for drug sensitivity test recommended by CLSI,micro broth dilution method was used to test the resistance to 15kinds of commonly used antibiotics for 119shigella strains isolated from feces of diarrhea patients.Results From 2015to 2017,a total of 119shigella strains were resuscitated and confirmed,all with drug resistance rate of 100.0%.Among different antibiotics, the resistance rate to erythromycin was the highest(95.8%),followed by naphthoic acid(90.8%),ampicillin(87.4%)and tetracycline(76.5%).There were 109multi-drug resistant strains,the multi-drug resistance rate was 91.6%.The maximum drug tolerance was resistant to 9kinds of antibiotics for 1strain.The drug resistance rates to tetracycline,cefoxitin,ampicillin/ sulbactam and erythromycin of different years were significantly different(all P<0.05).Conclusion The drug resistance of Shigella isolated from foodborne diseases serveillane in Jiangsu province was serious.It is necessary to further strengthen surveillance in order to understand the resistance of pathogens timely and guide rational drug use in clinic treatment.
