闭合撬拨复位联合螺钉三角状内固定术治疗跟骨骨折

Treatment of calcaneal fracture with closed prying reduction combined with screw triangle internal fixation

目的:探究跟骨骨折患者行闭合撬拔复位联合螺钉内固定术治疗的临床效果.方法:择取2012年6月-2017年10月期间我院收治的68例跟骨骨折患者,进行随机分组,对照组和观察组,每组34例.对照组患者行切开复位钢板内固定术,观察组患者行闭合撬拔复位联合螺钉三角状内固定术,比较2组患者治疗前后Bohler角、Gissane角、跟骨长、宽、高度及术后并发症.结果:术前术后2组患者5项指标差异经比较均无统计学意义(P>0.05),但同组治疗后相比治疗前跟骨宽度明显下降,剩余4项指标均显著提升,数据差异有统计学意义(P<0.05).观察组患者并发症发生率8.82%,相比对照组29.41%显著更低(P<0.05).结论:闭合撬拔复位联合螺钉三角状内固定治疗跟骨骨折效果与切开法无明显差异,但可有效规避并发症风险,因此可在临床中进行推广.

Objective:To investigate the clinical effect of closed pry pull reduction combined with screw fixation in the treatment of calcaneal fractures.Methods:68 patients with calcaneal fracture admitted to our hospital from June 2012 to October 2017 were randomly divided into control group and observation group,34 cases in each group.The control group was treated with open reduction and plate internal fixation,while the observation group was treated with closed prying reduction combined with screw triangle internal fixation.The Bohler angle,Gissane angle,calcaneus length,width,height and postoperative complications were compared between the two groups before and after treatment.Results:There was no significant difference in 5 indexes between the two groups before and after operation(P>0.05),but the calcaneal width decreased significantly before treatment and the remaining 4 indexes increased significantly after treatment(P<0.05).The incidence of complications in the observation group was 8.82%lower than that in the control group(P<0.05),compared with 29.41%in the control group.Conclusion:Closed prying reduction combined with screw triangular internal fixation for calcaneal fracture had no significant difference from incision,but it could effectively avoid the risk of complications,so it could be popularized in clinical practice.
