精益管理在神经内科多重耐药管理的应用效果

Application effect of lean management in multi-drug resistance management of neurology department

目的探究神经内科多重耐药管理工作中开展精益管理的效果及意义。方法52例神经内科收治的患者,随机分为对照组和观察组,各26例。对照组行常规管理干预,观察组行精益管理干预。比较两组患者多重耐药菌检出种类、多重耐药菌感染预防措施执行情况及感染率。结果两组铜绿假单胞菌、大肠埃希菌、肺炎克雷伯菌、金黄色葡萄球菌、鲍曼不动杆菌检出率比较,差异均无统计学意义(P>0.05)。观察组隔离执行率96.15%、消毒执行率88.46%及健康宣教执行率92.31%均显著高于对照组的73.08%、61.54%、69.23%,多重耐药菌感染发生率3.85%低于对照组的23.08%,差异均有统计学意义(P<0.05)。结论针对神经内科多重耐药开展精益管理可有效降低患者感染率,因此该种管理模式值得在临床中进行推广。

Objective To investigate the effect and significance of lean management in multi-drug resistance management of neurology department. Methods A total of 52 patients in neurology department were randomly divided into control group and observation group, with 26 cases in each group. The control group received regular management intervention, and the observation group received lean management intervention.Comparison were made on types of multi-drug resistant bacteria detected, implementation of preventive measures for multi-drug resistant infections and infection rate between the two groups. Results Both groups had no statistically significant difference in detection rate of Pseudomonas aeruginosa, Escherichia coli, Klebsiella pneumoniae, Staphylococcus aureus and Acinetobacter baumannii(P>0.05). The observation group had significantly higher implementation rate of isolation as 88.46%, implementation rate of disinfection as 88.46%and implementation rate of health education as 92.31% than 73.08%, 61.54% and 69.23% in the control group,and lower incidence of multi-drug resistant bacterial infections as 3.85% than 23.08% in the control group.Their difference was statistically significant(P<0.05). Conclusion Lean management for multi-drug resistance in neurology department can effectively reduce the infection rate of patients, so this management mode is worth promoting in clinical practice.
