周围型肺癌的X线诊断与影像学动态分析

The X-ray Diagnosis and Imaging Dynamic Analysis of Peripheral Lung Cancer

目的:通过对周围型肺癌患者实施X线检查和影像动态分析,探究其诊断效果。方法:从来我院诊治的2016年1月-2018年12月的周围型肺癌患者中随机抽取48例,进行X线检查和CT检查,观察其临床诊断效果和影像学动态结果。结果:动态学结果发现有42例患者病程长短不一,病灶呈不同程度增大,出现不规则性或者圆形、椭圆形肿块,最小直径为2.5mm,最长直径为9.0mm,平均直径为(6.1±1.02)mm。其中有30例分叶征,空洞征有14例,16例毛刺征,9例斑点化钙化,14例胸膜萎缩征;所有病灶都为结节状,20例分叶征,13例细毛刺征,16例胸膜萎缩征;肺内的单发结节病灶直径在9~20mm范围内,平均直径为14.3±1.23mm。在48例患者中,有36例出现结节状,没有钙化,密度较为均匀,其中有14例患者病灶有小透光区。有18例病灶轮廓不清的患者,其中有13例出现细小毛刺征,16例胸膜萎缩征,6例表现斑片影,病灶边缘较为模糊,形态不规则,6例出现空洞征。结论:患者病灶直径在15mm以内,边缘有分叶征、胸膜萎缩征、毛刺等表现,内有空泡征,不伴随临床症状,提示为早期周围型肺癌。

Objective:To investigate the diagnostic effect of X-ray examination and dynamic imaging analysis of peripheral lung cancer patients.Methods:A total of 48 patients with peripheral lung cancer who were treated in our hospital from January 2016 to December 2018 were enrolled in this study.The X-ray examination and CT examination were performed to observe the clinical diagnosis and imaging dynamics.Results:The dynamics found that 42 patients had different disease durations,lesions increased to varying degrees,irregularities or round,elliptical masses,with a minimum diameter of 2.5 mm,a longest diameter of 9.0 mm,and an average diameter of(6.1±1.02)mm. There were 30 cases of lobulated sign,14 cases of stenosis,16 cases of burr sign,9 cases of spotted calcification,14 cases of pleural atrophy;all lesions were nodular,20 cases of lobulation,13 cases of fine burr.There were 16 cases of pleural atrophy;the diameter of single nodular lesions in the lung was in the range of 9~20 mm,and the average diameter was(14.3±1.23)mm.Among the 48 patients,36 had nodular shape,no calcification,and a relatively uniform density.Among them,14 patients had small light transmission areas.There were 18 patients with unclear lesions.Among them,13 had fine burr signs,16 had pleural atrophy,and 6 showed patchy shadows.The edges of the lesions were fuzzy and irregular in shape,and 6 cases had empty signs.Conclusion:The diameter of the lesion was less than 15 mm.There were signs of lobulation,pleural atrophy and burr on the edge.There were vacuolar signs in the left,which were not accompanied by clinical symptoms,suggesting early peripheral lung cancer.
