MRI评价孕酮对急性创伤性脑损伤的修复作用

摘要 目的通过MRI观察孕酮(progesterone, PROG)治疗前后急性颅脑损伤大鼠颅内创伤灶的变化,评价PROG的神经修复作用。方法选择63只雄性SD大鼠,随机分为对照组(NC组,7只)、创伤组(TB组,28只)、创伤+孕酮治疗组(TP组,28只),其中TB和TP组又分为创伤后3 h、24 h、1周、2周4个亚组,每个亚组7只。对照组和各实验组大鼠分别行MRI T2WI横轴位、矢状位扫描。测量实验组大鼠脑创伤灶体积,比较TB、TP各亚组大鼠脑创伤灶体积的差异。结果MRI显示实验组大鼠各时间点脑内出血水肿程度不同。创伤灶的体积在伤后逐渐增大,24 h达高峰,1周及2周后逐渐缩小。TP组创伤灶修复程度明显好于TB组,1周时组间差异有统计学意义(t=2.179,P=0.010)。结论 PROG能够促进血肿吸收、减轻水肿,从而促进创伤灶修复进程,MRI能够动态直观显示创伤灶的变化。 Objective To observe the changes of acute traumatic brain injury(TBI) lesions in rats before and after progesterone(PROG) treatment by MRI, and to study the nerve effect of PROG. Methods A total of 63 male SD rats were randomly divided into normal control group(NC group, n=7), traumatic brain group(TB group, n=28) and traumatic brain with PROG treatment group(TP group, n=28). TB group and TP group were divided into 4 subgroups according to the time points(3 h, 24 h, 1 week, 2 weeks) after trauma. All rats underwent MR axial and sagittal T2 WI scanning. The volumes of traumatic lesions in the experimental group were measured according to the manifestation of T2 WI. The volumes of traumatic lesions in each subgroup of TB and TP rats were compared. Results Rats in the experimental group were tested for different degrees of cerebral hemorrhage or edema in the acute phase of injury by MRI. The volume of the traumatic lesion gradually increased with time, peaked at 24 h, and gradually decreased after one week and two weeks. The degree of repair in TP group was significantly better than that in TB group, and the difference between the two groups was significant at one week after trauma(P <0.05). Conclusions PROG can reduce the volume of traumatic lesion in rats with acute brain injury, reduce bleeding and improve edema, and promote the repair process of traumatic lesion. MRI can dynamically and visually show the changes of the traumatic lesion.
