舒芬太尼与芬太尼用于腰硬联合阻滞麻醉分娩镇痛的效果比较

Comparison of effects of sufentanil and fentanyl in labor analgesia under combined spinal-epidural anesthesia

目的对比分析芬太尼、舒芬太尼应用于腰硬联合阻滞麻醉分娩镇痛中的实际价值。方法选取滨州市中心医院2015年2月至2018年12月收治的120例自然分娩产妇作为研究对象,采用随机数字表法分为对照组与研究组,每组60例。研究组行舒芬太尼腰硬联合阻滞麻醉,对照组行芬太尼腰硬联合阻滞麻醉。对比分析芬太尼、舒芬太尼应用于腰硬联合阻滞麻醉分娩镇痛中的实际价值。结果研究组镇痛后至首次加药间隔时间显著长于对照组(P<0.05);两组T1时VAS法评分比较,差异未见统计学意义(P>0.05)。T2、T3、T4时研究组VAS法评分较T1下降幅度优于对照组(P<0.05);研究组分娩镇痛相关不良反应发生率(10.00%)低于对照组(21.67%),差异有统计学意义(P<0.05);两组新生儿Apgar评分比较,差异未见统计学意义(P>0.05)。结论应用舒芬太尼行腰硬联合阻滞分娩镇痛有效性、安全性均较优,有利于保障母婴生活质量及身心健康。

Objective To compare and analyze the practical value of fentanyl and sufentanil in labor analgesia under combined spinal-epidural anesthesia. Methods A total of 120 parturients with natural delivery admitted in Binzhou Central Hospital from February 2015 to December 2018 were selected as the study subjects. Random number table method was used to divide the patients into control group and research group, with 60 cases in each group.The study group received sufentanil combined spinal-epidural anesthesia, and the control group received fentanyl combined spinal-epidural block anesthesia. The actual value of fentanyl and sufentanyl in labor analgesia with lumbar epidural anesthesia were compared and analyzed. Results The interval between analgesia and initial administration in the study group was significantly longer than that in the control group (P<0.05), The VAS score of the two groups at T1 was significantly longer than that in the control group (P>0. 05). At T2, T3 and T4, the VAS score in the study group decreased more than that at T1 in the control group (P<0.05). The incidence of adverse reactions related to labor analgesia in the study group (10.00%) was lower than that in the control group (21.67%), and the difference was statistically significant (P<0.05). There was no significant difference in Apgar score between the two groups (P>0.05). Conclusions Sufentanil combined with spinal and epidural block is effective and safe to protect the quality of life and physical and mental health of mother and child.
