单中心腹腔镜与开腹直肠癌根治术的近远期疗效比较

Comparative study of short- and long-term clinical effects of single center laparoscopic radical resection and open radical resection on rectal cancer

目的比较开腹直肠癌根治术与腹腔镜直肠癌根治术的近远期疗效及安全性。方法选取2016年6月至2018年6月于临汾市人民医院进行手术治疗的150例直肠癌患者,根据手术方案将其分为两组。观察组75例采取腹腔镜直肠癌根治术治疗,对照组75例采取开腹直肠癌根治术治疗。比较两组手术指标、术中及术后并发症发生率。对两组病理学资料、复发率及远处转移率进行比较分析。结果观察组手术时间长于对照组,出血量少于对照组,排气恢复、引流管拔除、术后住院时间短于对照组(P<0.05)。两组术中及术后并发症发生率、阳性淋巴结及清扫淋巴结数目、环周切缘阳性率、全直肠系膜切除完整率、复发率比较差异均未见统计学意义(P均>0.05)。结论腹腔镜直肠癌根治术能够取得良好的近、远期疗效,治疗效果基本与开腹手术相当,具有可行性。

Objective To compare the short- and long-term clinical effects of laparoscopic radical resection and open radical resection on rectal cancer. Methods A total of 150 patients with rectal cancer hospitalized in Linfen People’s Hospital from June 2016 to June 2018 were selected and divided into two groups by surgery protocols. Seventy-five patients in observation group were treated by laparoscopic radical resection of colorectal cancer, and 75 patients in control group were treated by open radical resection of rectal cancer. The operation indexes, intraoperative and postoperative incidence of complications were compared between the two groups. The pathological data, recurrence rate and metastasis rate of the two groups were analyzed and compared. Results The operative time of observation group was longer than that of control group, the bleeding volume in observation group was less than that in control group, while the time of exhaust gas recovery, drainage extubation and the postoperative hospital stay in observation group was shorter than those in control group (P<0.05). There was no significant difference in the incidence of intraoperative and postoperative complications, the number of positive and dissected lymph nodes, the positive rate of circumferential resection margin, the complete rate of total mesorectal excision, or the recurrence rate between the two groups (P>0.05). Conclusions Laparoscopic radical resection of colorectal cancer is a feasible method, which can obtain good short-and long-term curative effect similar to open surgery.
