PCT联合WBC及CRP在神经外科术后感染诊断的应用

The value of procalcitonin,C-reactive protein and white blood cells in the diagnosis of neurosurgical postoperative infection

摘要 目的 通过分析神经外科患者术后PCT、WBC及CRP的动态变化,为临床及时准确诊断术后感染提供科学依据.方法 观察77例神经外科手术后患者,分析术后一周内的PCT、WBC及CRP的动态变化,明确其变化与手术应激、细菌感染的关系.结果 术后PCT均值0.282ng/mL,低于细菌感染标准0.5ng/mL,11.7%(9/77)提示发生细菌感染;WBC均值13.06×10^9/L,70.5%(43/61)高于参考上限;CRP均值49.9mg/L,82.4%(61/74)高于参考上限.三者阳性率差异有统计学意义(P<0.01),术后结果高低不一.结论 PCT、WBC和CRP结合运用有助于鉴别术后感染. Objective By analyzing the dynamic changes of postoperative PCT,WBC and CRP in neurosurgical patients,to provide the scientific basis for timely and accurate diagnosis of postoperative bacterial infection.Methods The dynamic changes of PCT,WBC and CRP in 77 patients after neurosurgery were observed,and the relationship between the changes and surgical stress and bacterial infection was determined.Results The postoperative mean PCT value was 0.282 ng/mL,lower than the standard of bacterial infection 0.5 ng/mL,and 11.7%(9/77) indicated bacterial infection.WBC mean 13.06 ×10^9/L,70.5%(43/61) above the reference upper limit.The mean CRP was 49.9 mg/L,82.4%(61/74) higher than the reference upper limit.The difference of positive rate among the three groups was statistically significant(P<0.01),and the results were inconsistent.Conclusion The combination of PCT,WBC and CRP helps to identify postoperative infections.
