血清NT-proBNP和PCT检测对慢性心力衰竭诊治的临床价值

Clinical value of serum NT-proBNP and PCT detection in the diagnosis and treatment of chronic heart failure

目的:探讨NT-proBNP(N端脑钠肽前体)和PCT(降钙素前体)在慢性心力衰竭(HF)诊断和治疗中的临床价值.方法:选取2017年10月~2018年10月山东齐鲁医院青岛院区住院的130例心力衰竭患者(实验组),及来自于体检中心的98名健康体检者(对照组),分别测定两组心脏彩色多普勒指数、血浆NT-proBNP和PCT水平并观察治疗前后上述指标的变化.结果:实验组左室舒张末期容积、左室收缩率、NT-proBNP浓度和PCT浓度与健康对照组比较有显着性差异(P<0.05).心功能越差,NT-proBNP和PCT的血浆水平越高(F=596.032,113.48,P均<0.05).经有效治疗后,实验组不同心功能分级患者NT-proBNP和PCT水平均较治疗前明显下降(P<0.05).结论:NT-proBNP和PCT的检测能反应心力衰竭患者的严重程度及治疗效果,对心力衰竭患者的诊治有极重要的临床价值.

Objective: To investigate the clinical value of NT-proBNP (N-terminal pro-brain natriuretic peptide) and PCT (calcitonin precursor) in the diagnosis and treatment of chronic heart failure (HF). Methods: A total of 130 patients with heart failure (experimental group) and 98 healthy subjects from the physical examination center (control group) were enrolled in the Qingdao branch of Shandong Qilu Hospital from October 2017 to October 2018, Cardiac color Doppler index, serum NT-proBNP and PCT levels were measured and the changes in the above indicators before and after treatment were observed. Results: The left ventricular end-diastolic volume, left ventricular contraction rate, NT-proBNP concentration and PCT concentration in the experimental group were significantly different from those in the healthy control group (P<0.05). The higher serum levels of NT-proBNP and PCT were followed with worse cardiac function (F=596.032, P<0.05;F=113.48, P<0.05). After effective treatment, the NT-proBNP and PCT levels in the experimental group were significantly lower than those before treatment (P<0.05). Conclusion: The detection of NT-proBNP and PCT can reflect the severity and therapeutic effect of patients with heart failure, and has important clinical value in the diagnosis and treatment of patients with heart failure.
