疼痛手册在癌症患者家庭护理中的应用

Application of Pain Manual in Home Care for Cancer Patients

目的探讨疼痛手册在癌症患者家庭护理中的应用效果。方法:采用随机抽样的方法抽取2016年3月-2017年5月在海南医学院附属第一医院肿瘤内科住院的癌症患者60名作为研究对象,随机分为干预组和对照组各30例。对照组由管床护士实施癌痛常规护理及出院指导,干预组由管床护士对患者及家属进行有关疼痛手册内容的培训,出院后由患者和家属共同对疼痛情况、治疗护理情况及效果进行观察、评价,并记录在手册上。研究人员每周定时电话随访1次,了解疼痛手册记录相关内容,帮助患者及家属解决存在的问题。比较干预前、干预后3个月两组疼痛强度(采用疼痛评价量表)、止痛治疗依从性(止痛治疗依从性量表)。结果:干预3个月后,干预组疼痛强度得分为5.03±1.080,低于干预前6.67±0.942,差异有统计学意义(t=6.383,P<0.001);低于对照组6.60±0.814差异有统计学意义(t=7.312,P<0.001);干预组服药依从性得分12.07±1.285,高于干预前7.63±0.928,差异有统计学意义(t=16.173,P<0.001);高于对照组7.77±0.898,差异有统计学意(t=14.916,P<0.001)。结论:运用疼痛手册能有效提高癌症患者止痛治疗依从性,减轻疼痛,提高患者生活质量。

Objective To explore the application of home care patients with cancer pain manual effect. Methods affiliated Hospital of Hainan Medical College from March 2016 to May 2017 were selected as the research objects by random sampling method,and randomly divided into the intervention group and the control group with 30 cases each. In the control group,tubebed nurses carried out routine nursing care and discharge guidance for cancer pain;in the intervention group,tube-bed nurses trained patients and their families on the contents of pain manuals;after discharge,patients and their families jointly observed and evaluated the pain,treatment and nursing conditions and effects,and recorded them in the manual. The researchers conducted regular telephone follow-up once a week to understand the contents recorded in the pain manual and help patients and their families solve the existing problems. Compare of pain intensity (pain assessment scale) and analgesic treatment compliance (analgesic treatment compliance scale) between the two groups before and 3 months after intervention. Results After 3 months of intervention,the pain intensity score of the intervention group was 5.03±1.080,which was lower than the intervention group before (6.67±0.942),and the difference was statistically significant (t=6.383,P<0.001). The pain intensity score of the intervention group was lower than the score of the control group (6.60±0.814),the difference was statistically significant (t=7.312,P<0.001). The drug compliance score of the intervention group was 12.07±1.285,higher than that of the pre-intervention group 7.63±0.928,and the difference was statistically significant (t=16.173,P<0.001). The difference was statistically significant (t=14.916,P<0.001).Conclusion pain manual can effectively improve the compliance of cancer patients with analgesic treatment,alleviate pain,and improve the quality of life of patients.
