维生素D与肠易激综合征患者胃肠症状严重程度的相关性研究

Correlation between vitamin D and severity degree of gastrointestinal symptoms in patients with irritable bowel syndrome

目的探索肠易激综合征(irritable bowel syndrome,IBS)患者胃肠症状严重程度与维生素D之间是否存在可能的相关性。方法纳入119例研究对象,包括61例IBS患者(IBS组)和58例健康对照者(HC组)。采集每位研究对象血样进行25-羟基维生素D[25-hydroxyvitaminD,25-(OH)D]检测;对IBS患者胃肠症状进行视觉模拟评分(VAS),并采用IBS病情严重程度评分系统(IBS-SSS)和综合医院焦虑/抑郁情绪测定表(HADS)对IBS患者病情严重程度进行评估。结果腹泻型IBS(IBS-D)组的血清25-(OH)D水平较HC组低[(18.32±8.25)ng/ml比(27.36±8.79)ng/ml],差异有统计学意义(P<0.001)。IBS患者腹痛、腹胀、对大便性状不满意程度、总体胃肠症状VAS评分、IBS-SSS评分、HADS评分与血清25-(OH)D水平呈负相关。结论IBS-D患者血清25-(OH)D水平比健康者低。血清25-(OH)D水平与IBS患者胃肠症状严重程度呈负相关。血清25-(OH)D水平越低,患者胃肠症状越严重,IBS-SSS评分越高。

Objective To explore the possible correlation between severity degree of gastrointestinal symptoms and vitamin D in patients with irritable bowel syndrome(IBS). Methods A total of 119 subjects were enrolled in this study,including 61 IBS patients (IBS group) and 58 healthy controls (HC group). Blood samples were drawn from all the participants for serum 25-hydroxyvitamin D [25-(OH)D] testing. Gastrointestinal symptoms were evaluated by visual analogue scale (VAS) for patients with IBS. We evaluated the condition of IBS patients by the IBS Severity Scoring System (IBS-SSS) and the Hospital Anxiety/Depression Scale (HADS). Results The serum 25-(OH)D concentration in diarrhea type IBS group(IBS-D)was lower than that in HC group [(18.32±8.25) ng/ml versus (27.36±8.79) ng/ml],and the difference was statistically significant ( P <0.001). The abdominal pain,abdominal bloating,dissatisfaction with stool traits,VAS score of overall gastrointestinal symptoms,the IBS-SSS score and the HADS score were negatively correlated with serum 25-(OH)D concentration in IBS patients. Conclusion Patients with IBS-D have lower serum 25-(OH)D concentration than healthy individuals. Serum 25-(OH)D concentration was negatively correlated with the severity degree of gastrointestinal symptoms in patients with IBS. The lower the serum 25-(OH)D concentration is,the higher the severity of the gastrointestinal symptoms is and the higher the IBS-SSS score is.
