血栓弹力图对心脑血管疾病患者抗血小板治疗效果的评价作用

Application of thromboelastography in antiplatelet therapy for patients with cardiovascular and cerebrovascular diseases

目的探讨血栓弹力图(thrombelastography,TEG)对心脑血管疾病患者抗血小板治疗效果的评价作用。方法选取山东省烟台毓璜顶医院2016年4月至2016年12月收治的100例心脑血管疾病患者为研究对象,按随机排列表法分为对照组和观察组,每组50例。对照组给予阿司匹林治疗,观察组给予阿司匹林联合氯吡格雷治疗,比较两组的TEG相关参数、血小板抑制率和药物疗效。结果对照组的R值[(4.28±1.26)min]和K值[(1.78±0.67)min]低于观察组[分别为(5.32±1.34)min、(2.35±1.03)min],Angle角[(65.35±6.23)deg]和MA值[(61.46±5.29)min]高于观察组[分别为(61.48±8.36)deg、(58.43±6.02)min],差异有显著性(P<0.05)。对照组的血小板抑制率为(75.32±14.74)%,低于观察组的(81.85±14.87)%。观察组药物总有效率(82.0%)高于对照组(68.0%),差异均有显著性(P<0.05)。结论 TEG对心脑血管疾病患者的抗血小板治疗具有较好的指导作用,可以有效预防和减少心脑血管疾病患者的血栓形成和出血风险,值得临床推广应用。

Objective To study the effect of thrombelastography(TEG)in detecting the risk of thrombosis and hemorrhage in patients with cardiovascular and cerebrovascular diseases.Method 100 patients with cardiovascular and cerebrovascular diseases admitted to Yuhuangding Hospital of Yantai City,Shandong Province,from April 2016 to December 2016,were selected as the research objects.They were divided into control group and observation group according to random arrangement table.50 patients in each group were treated with aspirin,while 50 patients in control group were treated with aspirin and clopidogrel.The observation group was treated with aspirin and clopidogrel.The TEG related parameters,platelet inhibition rate and drugs of the two groups were compared.The therapeutic effects were compared.Result The R value[(4.28±1.26)/min]and K value[(1.78±0.67)/min]of the control group were lower than those of the observation group[(5.32±1.34)/min,(2.35±1.03)/min],the Angle angle[(65.35±6.23)/deg]and MA value[(61.46±5.29)/min]were higher than those of the observation group[(61.48±8.36)/deg,(58.43±6.02)/min],and the platelet inhibition rate[(75.32±14.74)%]of the control group was lower than that of the observation group[(81.85±14.87)%].The total effective rate(82.0%)of the group was higher than that of the control group(68.0%).The difference was significant(P<0.05).Conclusion Thromboelastography has a good guiding effect on the medication of patients with cardiovascular and cerebrovascular diseases.It can effectively prevent and reduce the risk of thrombosis and hemorrhage in patients with cardiovascular and cerebrovascular diseases.It is worthy of clinical application.
