某院门诊麻醉药品的使用及其不合理用药处方的分析

To Investigate and Analyze the Use of Narcotic Drugs and Irrational Prescriptions of Narcotic Drugs in Outpatients in A Hospital

目的调查分析本院门诊患者麻醉药品的使用与不合理用药处方情况.方法回顾性分析本院门诊2016年~2017年度患者麻醉药品的具体使用情况,并展开麻醉药物不合理用药处方分析.结果①本院门诊注射类麻醉药物中以盐酸吗啡注射液用药量最多;口服类麻醉药物中以盐酸吗啡缓释片用药量最多.②注射类:氯胺酮注射液用药频度(DDDs)降低,盐酸吗啡注射液、枸橼酸芬太尼注射液、瑞芬太尼注射液DDDs上升;口服类麻醉药除了盐酸吗啡片其DDDs均增加,透皮贴剂的DDDs增加.③瑞芬太尼注射液、氯胺酮注射液、盐酸吗啡片的利用指数(DUI)均<1,盐酸吗啡注射液、枸橼酸芬太尼注射液、盐酸吗啡缓释片、盐酸羟考酮缓释片、磷酸可待因、芬太尼透皮贴剂DUI均>1.④枸橼酸芬太尼注射液、瑞芬太尼注射液的不合理用药处方数量呈上升趋势.结论本院门诊2016年~2017年度麻醉药物使用基本合理,但依旧需要加强用药监督与管理,促进药物使用更科学、合理.

Objective To investigate and analyze the use of narcotic drugs and irrational prescriptions of narcotic drugs in outpatients of our hospital. Methods The specific use of narcotic drugs in -2017 patients in our hospital in 2016 was analyzed retrospectively, and the irrational drug use prescription was analyzed. Results (1)The dosage of morphine hydrochloride injection was the most used in the anesthetic drugs in our hospital, and the dosage of MOrphine Hydrochloride Sustained-release Tablets was the most in oral anesthetic drugs.(2)injection: ketamine injection frequency (DDDs) decreased, morphine hydrochloride injection, fentanyl citrate injection and remifentanil injection DDDs increased, the DDDs of the oral anesthetic drugs were increased in addition to morphine hydrochloride tablets outside, the DDDs of transdermal patche increased.(3)The use index of remifentanil injection, ketamine injection and morphine hydrochloride tablets (DUI) was <1, morphine hydrochloride injection, fentanyl citrate injection, mOrphine hydrochloride sustainedrelease tablets, oxycodone hydrochloride prolonged-release tablets, codeine phosphate and fentanyl transdermal system DUI were >1.(4)The number of unreasonable prescriptions for fentanyl citrate injection and remifentanil injection showed an upward trend. Conclusion The use of narcotic drugs in our hospital 2016-2017 is basically reasonable, but it still needs to strengthen the supervision and management of drug use, and promote more scientific and reasonable usage of drugs.
