妊娠期糖尿病患者血糖水平变化与不良妊娠结局的关系研究

Study on the Relationship between the Changes of Blood Sugar Level and Adverse Pregnancy Outcomes in Patients with Gestational Diabetes Mellitus

目的探讨妊娠期糖尿病(GDM)患者血糖水平变化与不良妊娠结局的关系。方法将2016年1月至2018年9月入住我院的60例GDM患者作为研究组,将同期体检正常的60例产妇作为对照组,检测两组产妇清空腹血糖(FBG)及餐后2 h血糖(PBG)水平,对比分析两组产妇不良妊娠结局,分析FBG、PBG水平与不良妊娠结局关系。结果研究组FBG、PBG水平显著高于对照组(P<0.05);研究组胎儿窘迫、早产、胎膜早破、产后出血等不良妊娠结局的发生风险均显著高于对照组(P<0.05);通过Spearman分析,FBG及PBG水平变化与胎儿窘迫、早产、胎膜早破、产后出血均存在显著正相关性(P<0.05)。结论 GDM患者FBG及PBG水平显著升高,可增加产妇多种不良妊娠结局发生风险。

Objective To investigate the relationship between the changes of blood sugar level and adverse pregnancy outcomes in patients with gestational diabetes mellitus (GDM). Methods 60 GDM patients admitted to our hospital from January 2016 to September 2018 were taken as study group, and 60 normal pregnant women in the same period were taken as control group. The levels of FBG and PBG of the two groups were measured. The adverse pregnancy outcomes of the two groups were compared and analyzed. The relationship between FBG and PBG levels and adverse pregnancy outcomes was analyzed. Results The levels of FBG and PBG in the study group were significantly higher than those in the control group (P <0.05). The risk of adverse pregnancy outcomes such as fetal distress, premature delivery, premature rupture of membranes and postpartum hemorrhage in the study group was significantly higher than that in the control group (P <0.05). Spearman analysis showed that the changes of FBG and PBG levels were positively correlated with fetal distress, premature delivery, premature rupture of membranes and postpartum hemorrhage (P <0.05). Conclusion The levels of FBG and PBG in GDM patients are significantly increased, which can increase the risk of multiple adverse pregnancy outcomes.
