舒芬太尼联合罗哌卡因椎管内麻醉在无痛分娩中的应用效果分析

Analysis of the Application Effect of Sufentanil Combined with Ropivacaine for Spinal Anesthesia in Painless Delivery

目的分析舒芬太尼联合罗哌卡因椎管内麻醉在无痛分娩中的应用效果。方法方便选择2017年11月—2018年11月该院收治的分娩产妇84例,依据随机原则将其划分两组,设为研究组(42例)和对照组(42例),分别实施舒芬太尼联合罗哌卡因椎管内麻醉、常规分娩,对比产程时间、疼痛评分、新生儿Apgar评分、最终分娩方式和C反应蛋白。结果研究组产妇产程时间(571.6±33.3)min短于对照组(599.5±40.6)min(t=3.4434,P<0.05);研究组疼痛评分(5.5±0.6)分和C反应蛋白水平(8.6±2.6)mg/L低于对照组(9.7±0.2)分、(10.9±3.3)mg/L,新生儿Apgar评分(9.5±0.3)分显著高于对照组(9.0±0.1)分(t=43.0371、3.5479、10.2469,P<0.05);研究组自然分娩率(92.86%)显著高于对照组(76.19%),(χ^2=4.4594,P<0.05)。结论在无痛分娩中应用舒芬太尼联合罗哌卡因椎管内麻醉可将产程缩短,获取满意的镇痛效果,同时可将炎性反应降低,使母婴安全得以保证。

Objective To analyze the effect of sufentanil combined with ropivacaine for spinal anesthesia in painless delivery. Methods Eighty-four women who gave birth from November 2017 to November 2018 were convenient enrolled. According to the random principle, they were divided into two groups. The study group (42 cases) and the control group (42 cases), treated with sufentanil combined spinal anesthesia with ropivacaine, routine delivery respectively, versus labor time, pain score, neonatal Apgar score, final delivery mode, and C-reactive protein were compared. Results The maternal labor time of the study group (571.6±33.3) min was shorter than that of the control group (599.5±40.6) min,(t=3.443 4, P<0.05). The pain score of the study group (5.5±0.6) and C-reactive protein level (8.6±2.6) mg/L was lower than the control group (9.7±0.2),(10.9±3.3)mg/L, and the neonatal Apgar score (9.5±0.3)points was significantly higher than the control group (9.0±0.1)points,(t=43.037 1, 3.547 9, 10.246 9, P<0.05);the natural delivery rate of the study group (92.86%) was significantly higher than the control group (76.19%)(χ^2=4.459 4, P<0.05). Conclusion The application of sufentanil combined with ropivacaine for spinal anesthesia in painless delivery can shorten the labor process, obtain satisfactory analgesic effect, and reduce the inflammatory response, so that maternal and child safety can be guaranteed.
