柴胡左金汤辨证治疗胃食管反流病疗效研究

Clinical effect on patients with gastroesophageal reflux disease treated with Chaihu Zuojin decoction

目的:评价柴胡左金汤辨证治疗胃食管反流病的临床疗效。方法:收集胃食管反流病患者60例随机分为治疗组、对照组,治疗组采用柴胡左金汤辨证化裁治疗,对照组采用雷贝拉唑钠肠溶片口服治疗。两组治疗疗程均为8周,比较两组治疗前后症状积分变化以及停药2个月后复发率。结果:两组治疗前反酸、烧心、嗳气、胃脘胀满症状积分无统计学差异(P>0.05)。治疗后两组上述症状积分均有不同程度下降(P<0.05),但治疗组较对照组在反酸、烧心、胃脘胀满改善更明显(P<0.05)。治疗组总有效率96.67%,高于对照组80.00%,差异有统计学意义(P<0.01)。在随访期结束治疗组有4例复发,复发率13.33%;对照组有12例复发,复发率为40.00%,两组复发率比较,差异有统计学意义(P<0.05)。结论:中药柴胡左金汤辨证治疗胃食管反流病疗效确切,可有效改善反酸、烧心、嗳气、胃脘胀满症状,改善反流现象。

Objective :To investigate the clinical effect of Chaihu Zuojin decoction in the treatment of gastroesophageal reflux disease(GERD).Methods:60 cases of GERD collected according to the diagnostic criteria and inclusion criteria,were randomly divided into two groups. The treatment group( n =30) was treated with Chaihu Zuojin decoction,and the control group( n =30) was treated with rabeprazole sodium enteric-coated tablet(20mg/d). The course of treatment was 8 weeks.The changes of symptom score and recurrence rate after 2 months of with drawal were compared between the two groups before and after treatment. Results:There was no significant difference in symptom scores of acid reflux,heartburn,belching and epigastric fullness before treatment between the two groups( P <0.05).After treatment,the scores of the above symptoms in both groups decreased in varying degrees( P <0.05).The improvement of acid reflux,heart burn and epigastric fullness in the treatment group was more obvious than that in the control group( P <0.05).The total effective rate of the treatment group was 96.67%,higher than that of the control group (80.00%, P <0.01). At the end of the follow-up period,there were 4 recurrences in the treatment group (13.33%) and 12 recurrences in the control group (40.00%). There was a significant difference in the recurrence rate between the two groups( P <0.05).Conclusion:Chinese medicine Chaihu Zuojin decoction has definite curative effect on gastroesophageal reflux disease. It can effectively improve the symptoms of acid reflux,heartburn,belching and epigastric fullness,and improve reflux phenomenon.
