中医外治法在腰椎间盘突出症急性期的应用进展

Clinical research progress of external therapy of TCM for acute lumbar disc herniation

腰椎间盘突出症(LDH),是因腰椎间盘变性、纤维环破裂、髓核组织突出压迫和刺激腰骶神经根、马尾神经所引起的一种综合征,常发生于青、中年,好发部位为L4/5,L5/S1。临床多见患者以腰腿痛就诊,急性期患者常体现剧烈疼痛,活动受限且诉求直接,临床医生在诊疗手段的选择方面易受多方面因素限制,往往选择建议患者卧床休息及口服或注射西药治疗。中医理论指导下的多种类型外治疗法全面多样,对于急性疼痛治疗手段疗效确切,预后较好,且与常规治疗结合具有疗效增益作用。近年来中医外治法在现有基础上不断完善发展,对LDH急性期的治疗受到广泛肯定。本文将中医外治法在腰椎间盘突出症急性期的应用研究进展汇总整理,并对今后研究加以展望。

Lumbar disc herniation (LDH) is a syndrome caused by lumbar disc degeneration,rupture of annulus fibrosus,nucleus pulposus protrusion and stimulation of lumbosacral nerve roots,cauda equina,often occurs in young and middle-aged,and the predilection is L4./5,L5/S1. More common clinical patients with low back pain,acute patients often show severe pain,limited mobility and straightforward appeal. Clinicians are susceptible to many factors in the choice of diagnosis and treatment,often choose to recommend bed rest and oral or injection of non-steroidal anti-inflammatory drugs. The various types of external treatments under the guidance of TCM theory are comprehensive and diverse. The treatment of acute pain is effective and has a good prognosis. Combined with conventional treatment,it has an effect of increasing the curative effect. In recent years,the external treatment of Chinese medicine has been continuously improved on the existing basis,and the treatment of acute phase of LDH has been widely recognized. In this paper,the application of TCM external treatment in the acute phase of lumbar disc herniation is summarized. Propose the future research direction.
