改良抗酸染色联合CPA在基层医院诊断肺结核中的应用价值 被引量:1

摘要 目的分析改良抗酸染色联合交叉引物恒温扩增(CPA)在基层医院诊断肺结核中的临床应用价值。方法选择经临床确诊的45例肺结核患者,痰涂片镜检阴性标本34例,阳性标本11例。均分别采用Xpert MTB/RIF技术、CPA技术联合改良抗酸染色法、CPA技术、改良抗酸染色法检验痰液标本,比较4种方法的阳性检验结果。结果 45例标本,改良抗酸染色法阳性率为33.33%,明显低于CPA技术联合改良抗酸染色法(62.22%)、CPA技术(53.33%)、Xpert MTB/RIF技术(64.44%)( P <0.05)。结论 4种方法均对肺结核疾病有辅助临床诊断的作用。改良抗酸染色法联合CPA可节约检验成本,可代替Xpert MTB/RIF检测方式,适合在基层医院使用。 Objective To analyze the clinical value of modified acid-fast staining combined with crossing priming amplification(CPA) in the diagnosis of pulmonary tuberculosis in primary hospitals. Methods Forty-five patients with pulmonary tuberculosis were selected in this study.Sputum smear microscopy showed positive result in 11 patients and negative result in 34 patients.The sputum specimens were examined by Xpert MTB/RIF,CPA,improved acid-fast staining and CPA combined with improved acid-fast staining,respectively.The positive findings were compared among the four methods. Results The positive rate of modified acid-fast staining(33.33%) was lower than that of CPA(53.33%),Xpert MTB/RIF(64.44%) or CPA combined with modified acid-fast staining(62.22%)( P <0.05). Conclusion The Xpert MTB/RIF,CPA,improved acid-fast staining and CPA combined with improved acid-fast staining can assist in the diagnosis of pulmonary tuberculosis.The combination of improved acid-fast staining and CPA can save costs,replace Xpert MTB/RIF,and thus is suitable for use in primary hospitals.
