同伴教育对慢性乙型肝炎患者口服抗病毒药物治疗依从性的影响 被引量:23

摘要 目的探讨同伴教育对慢性乙型肝炎患者口服抗病毒药物依从性的影响。方法选择2015年6月-2017年12月口服抗病毒药物的慢性乙型肝炎患者90例,根据纳入的顺序进行编号,采用随机数字表法分为观察组和对照组各45例。观察组实施同伴教育,干预时间为住院到出院后3个月;对照组实施常规护理。对2组患者的心理压力、相关疾病及用药知识掌握程度及出院后服药依从情况进行比较。结果慢性乙型肝炎患者均存在较大心理压力水平,出院时及出院后3个月末观察组心理压力得分均明显低于对照组,差异有统计学意义(P<0.05);2组慢性乙型肝炎患者相关疾病及用药知识掌握程度得分比较差异有统计学意义(F=20.800,P<0.001);2组慢性乙型肝炎患者服药依从得分组间差异有统计学意义(F=16.071,P<0.001);观察组患者出院后1个月末、3个月末相关疾病及用药知识掌握程度得分高于对照组(均P<0.05);观察组患者出院后第2个月、第3个月服药依从得分高于对照组(均P<0.05)。结论长期口服抗病毒药物的慢性乙型肝炎患者存在一定心理压力,同伴教育可有效降低患者心理压力程度,使慢性乙型肝炎患者更好的掌握疾病治疗用药知识,减少不良服药行为。 Objective To explore the effect of peer education on medication compliace of oral antiviral drugs of patients with chronic hepatitis B. Methods Ninety chronic hepatitis B patients taking antiviral drugs orally from June 2015 to December 2017 were randomly divided into observation group with peer education(n=45) and control group with routine nursing(n=45). Intervention time was from admission to 3 months after discharge. Psychological pressure, knowledge on related disease and medication and medication compliance after discharge were compared in the two groups. Results It was common to observe psychological pressure among patients with chronic hepatitis B but the score of psychological pressure in the observation group was significantly lower than that in the control group in discharge and at the end of 3 months of discharge(P<0.05). The score of knowledge on related diseases and medication in the two groups showed statistical significance(F=20.800, P<0.001) and the score of medication compliance in the two groups was statistically significant(F=16.071, P<0.001). The score of knowledge on related diseases and medication in the observation group was higher at the end of the first and third month of discharge(all P<0.05) and the score of medication compliance in the observation group at the second and third month of discharge was higher(all P <0.05). Conclusion It is common to observe psychological pressure among patients with chronic hepatitis B taking antiviral drugs orally for a long time. Peer education is effective for alleviating psychological stress and improving knowledge on related diseases and medication and medication compliance.
