摘要
目的研究糖化血红蛋白(HbAlC)与尿微量白蛋白(U-mAlb)联合检验用于糖尿病肾病(DN)诊断的可行性。方法整理收集2016年4月—2018年4月我院收治的48例单纯糖尿病患者、36例糖尿病肾病(DN)患者以及40例健康对照组患者作为本次研究对象,所有研究对象均采集静脉血标本和尿标本,检测HbAlC、U-mAlb水平。结果 DN 组HbAlC、U-mAlb 水平高于单纯糖尿病组以及健康对照组,组间比较,差异具有统计学意义(P < 0.05)。单独检测HbAlC 对DN的敏感性、特异性分别为77.4%、81.0%,高于U-mAlb,而二者联合检测时,敏感性、特异性则会提高到88.1%、84.5%,组间比较,差异具有统计学意义(P < 0.05)。结论 HbAlC 联合U-mAlb 检测对诊断DN 的可行性高,两者联合测定可作为临床诊断糖尿病肾病的理想指标。
Objective To study the feasibility of combined detection of glycosylated hemoglobin (HbAlC) and urinary microalbumin (U-mAlb) in the diagnosis of diabetic nephropathy (DN). Methods From April 2016 to April 2018, 48 patients with simple diabetes mellitus, 36 patients with diabetic nephropathy (DN) and 40 healthy controls were collected as the subjects of this study. All subjects were collected venous blood and urine samples to detect the levels of HbAlC and U-mAlb. Results The levels of HbAlC and U-mAlb in DN group were significantly higher than those in diabetes mellitus group and healthy control group, the difference was statistically significant (P < 0.05). The sensitivity and specificity of HbAlC for DN were 77.4% and 81.0% respectively, which were higher than those of U-mAlb. When combined detection, the sensitivity and specificity of HbAlC were increased to 88.1% and 84.5% respectively, the difference was statistically significant (P < 0.05). Conclusion HbAlC combined with U-mAlb detection is highly feasible for the diagnosis of DN. The combined detection of HbAlC and U-mAlb can be used as an ideal index for clinical diagnosis of diabetic nephropathy.
