不同剂量右美托米啶在糖尿病湿性坏疽清除术中对循环系统以及患者睡眠状态的影响

Effects of Different Doses of Dexmetomidine on Circulatory System and Sleep Status of Patients During Clearance of Diabetic Wet Gangrene

目的:探讨不同剂量右美托米啶在糖尿病湿性坏疽清除术中对循环系统以及患者睡眠状态的影响。方法:选取2017年2月至2018年2月北京丰台南苑医院接受湿性坏疽清除术治疗的糖尿病患者73例作为研究对象。按照右美托米啶应用剂量不同分为低剂量组[0.5μg/(kg·h)](n=35)和高剂量组[1.0μg/(kg·h)](n=38)。观察2组患者手术期间循环系统变化。结果:1)术后,高剂量组SDANN(88.98±7.05)ms,RMSSD(39.84±5.15)ms,SDNN(95.27±5.68)ms,低剂量组SDANN(93.85±5.59)ms,RMSSD(44.71±3.06)ms,SDNN(106.27±5.42)ms,低剂量组和高剂量组各项指标组间比较,差异有统计学意义(P<0.05)。2)比较2组患者短期睡眠状况发现高剂量组(5.23±1.64)分对比低剂量组(5.34±1.81)分,差异无统计学意义(P>0.05)。结论:低剂量右美托米啶更适用于糖尿病湿性坏疽清除术的治疗,在维持患者机体循环系统稳定方面效果良好。

Objective:To investigate the effects of different doses of dexmetomidine on circulatory system and sleep status of diabetic patients during clearance of diabetic wet gangrene. Methods:A total of 73 cases were selected from February 2017 to February 2018 with diabetes mellitus who were given wet gangrene clearance in our hospital.According to the different doses of dexmetomidine,the patients were divided into low dose group [0.5 μg/(kg·h)]( n =35)and high dose group [1.0 μg/(kg·h)]( n =38).The changes of circulatory system during operation were observed in both groups. Results:1)After the surgery,in high dose group,the score of SDANN was(88.98±7.05)ms,the score of RMSSD was(39.84±5.15)ms ,the score of SDNN was(95.27±5.68)ms;in low dose group,the score of SDANN was(93.85±5.59)ms,the score of RMSSD was(44.71±3.06)ms ,the score of SDNN was(106.27±5.42)ms.There was no significant difference between the low dose group and the high dose group( P >0.05).2)When comparing the short-term sleep status of the two groups,there was no significant difference between the high-dose group(5.23±1.64)and the low-dose group(5.34±1.81).( P >0.05). Conclusion:For the wet gangrene clearance in diabetes mellitus, low dose of dexmetomidine is more suitable and has a good effect on maintaining the stability of the circulatory system in patients with diabetes mellitus.
