同质化救治流程对重症急性胰腺炎预后的影响

Effect of homogenization treatment process on the prognosis of severe acute pancreatitis

目的:探讨制定同质化救治流程对重症急性胰腺炎(severe acute pancreatitis,SAP)预后的影响。方法:选择2015-11-01~2018-11-30我科收治的急性胰腺炎患者64例,其中32例按照同质化流程方案治疗,设为同质化流程治疗组;32例按照《急性胰腺炎诊治指南(2014)》治疗,设为常规治疗组。比较两组的复苏达标时间、机械通气率、连续肾脏替代疗法(continuous renal replacement therapy,CRRT)治疗率、病死率、平均腹内压等临床指标。结果:同质化流程治疗组的12h复苏达标率、CRRT治疗率、病死率、平均腹内压与常规治疗组相比,差异有统计学意义(P<0.05)。结论:制定同质化救治流程,逐项按要求达标,可快速高效地达到复苏指标,显著降低SAP的机械通气率、CRRT治疗率、降低腹内压及病死率。

Objective:To investigate the effect of homogenization treatment process on the prognosis of severe acute pancreatitis(SAP).Methods:Sixty-four patients with acute pancreatitis from 2015-11-01 to 2018-11-30 were selected in this study,32 patients were treated according to the homogenization process(homogenization process treatment group),and 32 patients were treated according to the guidelines according to the routine(contrast group).Clinical indicators such as recovery time,mechanical ventilation rate,continuous hemofiltration(CRRT)rate,mortality and mean intra-abdominal pressure in the two groups were compared.Results:There were significant difference in the 12 hrecovery rate of the treatment group,CRRT rate,mortality rate and average intra-abdominal pressure between the homogenization process treatment group and contrast group.Conclusion:The homogenization treatment process is formulated,and the target can be achieved according to the requirements.It can quickly and efficiently achieve the recovery index,can significantly reduce the mechanical ventilation rate,CRRT rate of severe acute pancreatitis,the intra-abdominal pressure and the mortality.
