摘要
我国老龄化程度不断加快,老年群体健康管理与长期照护的需求日益强烈。文章针对目前老年群体健康管理与长期照护的发展现状,从内涵、流程、医学、信息技术等角度分析老年群体健康管理与长期照护一体化的合理性,并构建集信息收集与建档、评估分类、分级健康干预与分级长期照护、长期追踪、效果评价为一体的老年群体健康管理与长期照护一体化模式,最后提出一体化模式的配套机制。
With the acceleration of aging in China, the demand for health management and long-term care for the elderly is increasing. In view of the current situation of health management and long-term care for the elderly, this paper analyses the rationality of the integration of health management and long-term care for the elderly from the perspectives of connotation, process, medicine and information technology, and constructs a health management system for the elderly, which integrates information collection and filing, assessment and classification, graded health intervention and graded long-term care, long-term tracking and effect evaluation. Finally, the supporting mechanism of the integrated model is put forward.
