老年单纯收缩期高血压患者血清Hcy与颈动脉IMT的关系

The relationship between serum Hcy and carotid IMT in elderly patients with isolated systolic hypertension

目的观察老年单纯收缩期高血压患者血清同型半胱氨酸(Hcy)水平与颈动脉内膜中层厚度(IMT)的关系。方法选择2017年10月至2018年10月在本院门诊或住院且年龄≥60岁的单纯收缩期高血压患者118例(观察组),监测24h动态血压,根据4AM^8AM收缩压水平进行分级,同时选择血压正常者60例作为对照组。检测血清Hcy水平,超声测量颈动脉IMT,比较两组患者血清Hcy水平及颈动脉IMT,并分析其相关性。结果高血压组IMT(1.13±0.32)mm,较血压正常组IMT(0.65±0.07)mm增高,差异有统计学意义(t=11.461,P<0.05);高血压组Hcy水平(17.03±7.6)μmol/L与对照组(8.19±1.91)μmol/L比较差异有统计学意义(t=8.857,P<0.01)。随着晨间血压分级的增加,IMT和血清Hcy水平均有增高趋势,IMT与血清Hcy水平呈正相关(r=0.572,P<0.05)。结论老年单纯收缩期高血压患者血清Hcy增高,与颈动脉IMT相关。

Objective To evaluate the relationship between serum homocysteine(Hcy) and carotidintima-media thickness(IMT) in elderly patients with isolated systolic hypertension(ISH). Methods 118 elderly patients with ISH were enrolled from October 2017 to October 2018(ISH group). Ambulatory blood pressure was recorded and systolic blood pressure was graded based on blood pressure during 4 AM-8 AM. 60 persons with normal blood pressure were selected as control group. Serum Hcy was measured and carotid IMT was determined by ultrasonography. Hcy and IMT were compared between the two groups. The relation between Hcy and IMT was analyzed. Results IMT and Hcy were significantly higher in ISH group [(1. 13 ± 0. 32 mm,(17. 03 ± 7. 6) mmol/L] than in normal blood pressure group [(0. 65 ±0.07) mm,(8.19 ±1.91) mmol/L],(P< 0.05 or 0.01). IMT and serum Hcy increased gradually as the increase of blood pressure. IMT was positively correlated with serum Hcy level(r=0. 572, P < 0. 05). Conclusion Serum Hcy increases in elderly patients with ISH and relates to carotid IMT.
