奥沙利铂联合替吉奥治疗晚期结直肠癌的效果及对患者血清学指标的影响

Efficacy of Oxaliplatin Combined with Tegafur in the Treatment of Advanced Colorectal Cancer and Its Effect on Serum Markers

目的:讨论奥沙利铂联合替吉奥治疗晚期结直肠癌的效果及对患者血清学指标的影响。方法:选择2017年1月-2018年8月笔者所在医院晚期结直肠癌患者62例,根据治疗方式不同将其分为两组,每组31例。对照组患者使用奥沙利铂联合卡培他滨治疗,观察组患者使用奥沙利铂联合替吉奥治疗,治疗6个疗程后,对两组患者临床疗效、治疗期间毒副作用发生情况及治疗前后白细胞计数(WBC)、血小板计数(PLT)及miR-21表达水平进行比较。结果:两组患者客观缓解率、疾病控制率比较,差异均无统计学意义(P>0.05);治疗期间,观察组患者手足综合征、胃肠道反应发生率均明显低于对照组,差异有统计学意义(P<0.05);两组患者WBC、PLT、miR-21表达水平均较治疗前降低,且观察组miR-21表达水平明显低于对照组,差异有统计学意义(P<0.05)。结论:奥沙利铂联合替吉奥治疗晚期结直肠癌,安全性更高,miR-21表达水平降低更明显,对患者的预后意义重大,在临床上的应用价值高。

Objective:To discuss the efficacy of Oxaliplatin combined with Tegafur in the treatment of advanced colorectal cancer and its effect on serum markers.Method:62 patients with advanced colorectal cancer in our hospital from January 2017 to August 2018 were selected and divided into two groups according to the different treatment methods,31 cases in each group.Patients in the control group were treated with Oxaliplatin combined with Capecitabine,and patients in the observation group were treated with Oxaliplatin combined with Tegafur.After 2 courses of treatment,the clinical efficacy of the two groups,the occurrence of toxic side effects during treatment,the white blood cell count(WBC),platelet count(PLT) and miR-21 expression levels before and after treatment were compared.Result:There was no significant difference in objective response rate and disease control rate between the two groups(P>0.05).During the treatment period,the incidence rates of hand-foot syndrome and gastrointestinal reactions in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The expression levels of WBC,PLT and miR-21 in the two groups were lower than those before treatment,and the expression levels of miR-21 in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Oxaliplatin combined with Tegio in the treatment of advanced colorectal cancer is more safe,and the expression of miR-21 is decreased more significantly,which is of great significance to the prognosis of patients and has high clinical application value.
